摘要
目的了解甘南藏族自治州严重精神障碍的患病率和人口特征分布,为进一步开展严重精神障碍的防治工作提供基础数据。方法对国家严重精神障碍信息系统内的甘南州辖区严重精神障碍患者的基本信息和管理治疗情况,按照人口学特征和疾病特征采用SPSS19.0软件对样本进行统计分析。结果截至2018年3月31日,甘南州严重精神障碍确诊病例共1 385例,患病率为1.949‰,精神分裂症患病率为1.167‰,各类型患病率差异有统计学意义(χ~2=2 516.696,P<0.05),男女性别患病率差异无统计学意义(χ~2=2.440,P>0.05),汉族和少数民族发病率差异无统计学意义(χ~2=2.859,P>0.05),患者文化程度低,以农牧民为主,71.91%在贫困线以下;患者规范管理率、服药率、规律服药率均低于国家要求。结论甘南州严重精神障碍发病率处于较低水平,精神分裂症为管理治疗重点,应加强精神卫生防治队伍建设和农牧民群众、低文化程度、贫困人群严重精神障碍患者的政策帮扶。
Objective To examine the prevalence rate and distribution characteristics of severe mental disorders(SMDs)in Gannan Tibetan autonomous prefecture and provide the basis for further prevention of SMDs.Methods A survey was carried out among patients with SMDs who from Gannan region and had registered in the national SMDs data base,an analysis was conducted about these patients’basic information and treatment effects and SPSS 19.0 was used in date analysis according to the features of demography and diseases.Results By the end of March 31,2018,the total number of diagnosed SMDs in Gannan was 1 385 cases,the overall prevalence rate was 1.949‰,the prevalence rate of schizophrenia was 1.167‰,the difference in the prevalence rates of various SMDs was statistically significant(χ~2=2 516.696,P<0.05).There was no statistical difference between male and female(χ~2=2.440,P>0.05),but there was no significant difference between Han and ethnic minorities(χ~2=2.859,P>0.05).To illustrate,patients were almost poor-educated and the majority of them were farmers or herdsmen,71.91%of them lived below the poverty line.The rate of normative management,taking medicines,and taking medicines regularly were all below the national demands.Conclusion The prevalence rate of SMDs in Gannan is comparatively at a low level,schizophrenia is the key of management and treatment,the construction of mental health prevention and control team should be strengthened.In the future,we should strengthen the construction of mental health prevention and treatment teams and provide policy support to farms and herdsmen,poor people with low educational level and people with serious mental disorders.
