新生儿脓毒性休克早期血流动力学的特征 被引量:1

摘要 目的探讨新生儿脓毒性休克早期血流动力学的特征。方法将在新生儿科住院诊断休克的患儿分为观察组和对照组。诊断为脓毒性休克的病例纳入观察组,其余纳入对照组。比较两组心脏每搏输出量(SV)、心排出量(CO)、心指数(CI)、全身血管阻力(SVRI)差异,运用受试者工作特征图进行分析,计算敏感度和特异度。结果观察组8例,对照组20例。两组患儿胎龄、出生体重、性别比例和剖宫产率差异无统计学意义(P>0.05)。与对照组比较,观察组SV、CO和CI均明显升高[(2.70±1.43)mL/kg vs(1.84±0.52)mL/kg(P=0.025);(402.75±207.15)mL/(kg·min)vs(216.55±82.00)mL/(kg·min)(P=0.002);(5.48±2.81)mL/(min·m^2)vs(3.17±1.13)mL/(min·m^2)(P=0.004)),SVRI明显降低[(886.25±439.11)(dyn·s/cm5)·m^2vs(1606.35±792.47)(dyn·s/cm5)·m^2(P=0.023)]。当CO高于318 mL/(kg·min)时,诊断脓毒性休克的敏感度为90.0%,特异度为75.0%。结论新生儿脓毒性休克血流动力学特点是高排低阻,可以通过CO来判断脓毒性休克的发生,但其作为诊断的价值还需要进一步验证。
