全自动三维超声右室定量软件评价室性期前收缩患者右心室功能

Evaluation of right ventricular function by fully automated three-dimensional ultrasound right ventricle quantification software in patients with premature ventricular contractions

摘要 目的应用全自动三维超声右室定量软件(3D Auto RV)检测室性期前收缩(PVCs)患者右心室收缩功能,评价其临床应用价值。方法选取2021年10月至2024年5月于福州大学附属省立医院确诊的45例PVCs患者为PVCs组,45例健康体检者为对照组。应用飞利浦EPIQ CVX超声仪对患者进行检查,常规测量右房径(RAD)、右室径(RVD)、肺动脉内径(PA)、肺动脉收缩压(PASP)、右室面积变化率(RVFCA)、三尖瓣环收缩期偏移(TAPSE)、三尖瓣环收缩期峰值速度(S’)。观察3D Auto RV指标:右室收缩末容积指数(RVESVi)、右室舒张末容积指数(RVEDVi)、右室每搏输出量(RVSV)和右室射血分数(RVEF)。结果PVCs组与对照组的MPA、RVD、RAD、PASP、RVFAC、TAPSE、S’差异无统计学意义(P>0.05)。PVCs组的RVEDVi、RVESVi大于对照组,RVEF、RVSV小于对照组,差异有统计学意义(P<0.05)。结论3D Auto RV可以为PVCs患者提供额外的右心室功能评估信息,是右心功能评价的有效工具。 Objective To test the right ventricular systolic function in patients with premature ventricular complexes(PVCs)using the fully automated three-dimensional ultrasound right ventricular quantification software(3D Auto RV),and to evaluate its clinical application value.Methods A total of 45 patients diagnosed with PVCs at Fuzhou University Affiliated Provincial Hospital from October 2021 to May 2024 were selected as the PVCs group,and 45 healthy individuals confirmed by physical examinations were selected as the control group.A Philips EPIQ CVX ultrasound machine was utilized to examine the patients,and the following measurements were routinely taken:right atrial diameter(RAD),right ventricular transverse diameter(RVD),pulmonary artery internal diameter(PA),pulmonary artery systolic pressure(PASP),right ventricular area change rate(RVFCA),tricuspid annular plane systolic excursion(TAPSE),and tricuspid annular peak systolic velocity(S’).3D Auto RV indexes included right ventricular end-systolic volume index(RVESVi),right ventricular end-diastolic volume index(RVEDVi),and right ventricular stroke volume(RVSV)and right ventricular ejection fraction(RVEF).Results There was no statistically significant difference in MPA,RVD,RAD,PASP,RVFAC,TAPSE and S’between the PVCs group and the control group(P>0.05).RVEDVi and RVESVi were greater,and RVEF and RVSV were lower in the PVCs group than those in the control group,with statistically significant differences(P<0.05).Conclusion 3D Auto RV can provide additional information for right ventricular function assessment in patients with PVCs and is an effective tool for right ventric ular function evaluation.
