摘要
目的通过中英文文献系统评价胚胎培养液中线粒体DNA(mtDNA)拷贝数与体外受精-胚胎移植(IVF-ET)治疗中胚胎质量的相关性。方法检索中国知网、万方、维普等中文数据库和PubMed、Embase、the Cochrane Library等英文数据库中与胚胎培养液中mtDNA拷贝数和IVF-ET治疗中胚胎质量相关的临床研究,检索时限为建库至2023年12月。采用Cochrane系统评价文献质量,并提取文献中相关数据应用RevMan5.3软件进行分析。结果本研究共纳入15篇文献(8篇中文文献,7篇英文文献),包括3542例患者。Meta分析结果显示,接受IVF-ET治疗患者mtDNA拷贝数与受精率、优质胚胎率、胚胎评分呈强正相关性(r=0.726、0.668、0.709,P<0.00001),与卵裂率呈弱正相关性(r=0.280,P=0.030)。发表偏倚分析结果显示,mtDNA拷贝数与受精率、优质胚胎率、优质胚胎率、胚胎评分相关性文献的漏斗图分布点基本对称,无发表偏倚。结论胚胎培养液中mtDNA拷贝数与IVF-ET治疗中胚胎质量有一定相关性,临床可通过检测胚胎培养液中mtDNA拷贝数辅助评估胚胎质量,为临床IVF-ET治疗过程控制提供评定依据。
Objective To evaluate the correlation between the copy number of mitochondrial DNA(mtDNA)in embryo culture medium and embryo quality in in vitro fertilization-embryo transfer(IVF-ET)treatment through Meta-analysis of Chinese and English literatures systems.Methods The Chinese databases of CNKI,Wanfang,VIP and the English databases of PubMed,Embase and the Cochrane Library were searched for clinical studies on the relationship between the copy number of mtDNA in embryo culture medium and embryo quality in IVF-ET treatment.The search period was from the establishment of the databases to November 2023.The literature quality was evaluated by Cochrane system.The relevant data in the literature were extracted and analyzed by RevMan5.3 software.Results A total of 15 papers(8 Chinese papers and 7 English papers)were included in this study,including 3,542 patients.The results of Meta-analysis showed that the copy number of mtDNA in patients receiving IVF-ET treatment was highly positively correlated with the fertilization rate,high quality embryo rate and embryo score(r=0.726,0.668,0.709;P<0.000,01),and weakly positively correlated with the cleavage rate(r=0.280,P<0.030).The results of publication bias analysis showed that the funnel chart distribution of the correlation between the copy number of mtDNA and the fertilization rate,high quality embryo rate,high quality embryo rate and embryo score was basically symmetrical with no publication bias.Conclusions There is a certain correlation between the copy number of mtDNA in embryo culture medium and the embryo quality in IVF-ET treatment.Clinical evaluation of embryo quality can be assisted by detecting the copy number of mtDNA in embryo culture medium,which provides an evaluation basis for clinical process control of IVF-ET treatment.
