摘要
目的:探讨加温毯联合液体加温在输尿管镜钬激光碎石中的应用。方法:选取2023年11月~2024年4月在本院进行输尿管软镜钬激光碎石治疗的上尿路结石患者108例。依据患者入院的先后次序,平均将其划分为两组,试验组和对照组,各包括54例患者。在手术过程中,对照组使用的是常规温度的冲洗液和静脉输液;而试验组在此基础上,采用加温毯联合液体加温灌洗液和输注至体内的液体,将液体温度调节至37˚C。分析两组接受手术的患者,在手术前(t_(1))、手术进行至30min(t_(2))、手术完成时(t_(3))及手术后30min(t_(4))的平均血压、心跳次数及体温的波动,并调查手术期间出现的颤抖、焦躁症状、失血量,以及术后镇静水平(RASS评分)和生活品质问卷(SF-36评分)的结果。结果:试验组中的t_(1)、t_(2)、t_(3)、t_(4)在血压、心跳数、体温方面的比较均未表现统计学上的显著差异(P>0.05);而在对照组中,t_(2)、t_(3)、t_(4)的体温较t_(1)低,血压及心跳频率则较t_(1)高(P<0.05)。试验组在手术过程中出现的寒颤、不安行为、失血量以及RASS评估得分均明显少于对照组,且其SF-36评分较对照组高(P<0.05)。结论:在进行输尿管软镜钬激光打碎结石手术时,通过加温毯和液体加温的双重作用能够保持患者的生命指标稳定,减轻患者手术过程中的颤抖和焦虑情绪,这对于提升患者接受治疗时的感受是至关重要的。
Objective:To investigate the application of heating blanket combined with liquid heating in ureteroscopic holmium laser lithotripsy.Methods:From November 2023 to April 2024,108 patients with upper urinary tract calculi treated by flexible ureteroscopic holmium laser lithotripsy in our hospital were selected.According to the order of admission,the patients were divided into two groups,the experimental group and the control group,including 54 patients in each group.In the operation process,the control group used conventional temperature flushing fluid and intravenous infusion;On this basis,the experimental group used heating blanket combined with liquid heating lavage liquid and liquid infused into the body to adjust the liquid temperature to 37˚C.The fluctuations of blood pressure,heart rate and body temperature of two groups of patients undergoing surgery were analyzed before surgery(t_(1)),30 minutes after surgery(t_(2)),at the completion of surgery(t_(3))and 30 minutes after surgery(t_(4)),and the shivering,irritable symptoms,blood loss,postoperative sedation level(RASS score)and quality of life questionnaire(SF-36 score)were investigated.Results:The comparison of t_(1),t_(2),t_(3)and t_(4)in blood pressure,heart rate and body temperature in the experimental group showed no statistically significant difference(P>0.05);In the control group,the body temperature at t_(2),t_(3)and t_(4)was lower than that at t_(1),while the blood pressure and heart rate were higher than that at t_(1)(P<0.05).The shivering,restless behavior,blood loss and RASS scores in the experimental group were significantly lower than those in the control group,and the SF-36 score was higher than that in the control group(P<0.05).Conclusion:In the process of flexible ureteroscopic holmium laser lithotripsy,the dual effects of heating blanket and liquid heating can maintain the stability of the patient’s life indicators,reduce the shivering and anxiety of patients during the operation,which is very important for improving the patients’feelings during treatment.
