摘要
目的:比较MRI与数字化X射线摄影(DR)检查在膝关节隐匿性骨折诊断中的价值。方法:选取2023年1月—2023年6月临沂市郯城县第一人民医院收治的40例疑似膝关节隐匿性骨折并行手术治疗的患者作为研究对象,均采用MRI和DR检查,对比不同病变组织和类型的检出准确率,分析两种检查方法对膝关节隐匿性骨折的诊断效能。结果:MRI检查在骨小梁骨折、软骨骨折、滑膜损伤、半月板损伤等病变组织的检出准确率高于DR检查(P<0.05)。MRI检查对Ⅰ型、Ⅱ型和Ⅲ型隐匿性骨折的检出准确率高于DR检查(P<0.05)。MRI检查与DR检查诊断敏感度、特异度、误诊率、阳性预测值、阴性预测值、准确性比较,差异无统计学意义(P>0.05);MRI检查的漏诊率低于DR检查(P<0.05)。结论:与DR检查相比,MRI在膝关节隐匿性骨折的诊断中,对病变组织及骨折类型有更高的检出准确率,漏诊率较低,值得临床推广。
Objective:To compare the value of MRI and digital X-ray(DR)in the diagnosis of occult fracture of knee joint.Methods:Forty patients with suspected occult fracture of knee joint treated by Linyi Tancheng County First People's Hospital from January 2023 to June 2023 were selected as the study objects,all of whom were examined by MRI and DR.The accuracy of detection of different pathological tissues and types was compared,and the diagnostic efficacy of the two methods for occult fracture of knee joint was analyzed.Results:The accuracy of MRI in detecting bone trabecular fracture,cartilage fracture,synovial injury and meniscus injury was higher than that of DR examination(P<0.05).The accuracy of MRI in detecting typeⅠ,typeⅡand typeⅢconcealed fractures was higher than that of DR(P<0.05).There was no significant difference in diagnostic sensitivity,specificity,misdiagnosis rate,positive predictive value,negative predictive value and accuracy between MRI and DR(P>0.05);the missed diagnosis rate of MRI was lower than that of DR(P<0.05).Conclusion:Compared with DR examination,MRI in the diagnosis of occult fracture of knee joint has a higher detection accuracy for pathological tissue and fracture type,and a lower rate of missed diagnosis,which is worthy of clinical promotion.
