摘要
目的研究苦参汤加减联合西药治疗脾虚湿蕴型慢性湿疹患者的临床效果。方法选取浙江省温州市中医院2022年2月至2024年1月收治的192例脾虚湿蕴型慢性湿疹患者进行回顾性分析,按照不同治疗方式将接受常规西药治疗患者纳入西药组(89例),接受常规西药联合苦参汤加减治疗患者纳入联合组(103例),2组疗程均为4周,对比2组患者临床疗效、皮损程度[湿疹面积及严重程度指数(EASI)评分]、瘙痒程度[视觉模拟评分(VAS)]、外周血炎性因子水平及不良情况。结果联合组临床治疗总有效率为97.1%,高于西药组的88.8%(P<0.05);治疗后,联合组患者皮损面积、丘疹、表皮剥脱、苔藓化的EASI评分均低于西药组(P<0.05);治疗2周、4周后,联合组患者VAS评分均低于西药组(P<0.05);治疗后,联合组患者干扰素?鄄γ、白细胞介素?鄄4和C反应蛋白(CRP)水平均低于西药组(P<0.05);联合组组患者用药不良反应总发生率为(6.8%),与西药组(3.4%)对比差异无统计学意义(P>0.05)。结论苦参汤加减联合西药治疗脾虚湿蕴型慢性湿疹患者临床效果显著,能有效缓解患者皮损情况及瘙痒症状,改善炎症因子水平,且具有较好安全性,值得推广应用。
Objective To study the clinical effect and influence of modified Kushen decoction combined with western medicine in the treatment of patients with chronic eczema of spleen deficiency and dampness accumulation type.Methods A total of 192 patients with chronic eczema of spleen deficiency and dampness accumulation type in Wenzhou Traditional Chinese Medicine Hospital were selected from February 2022 to January 2024 and performed retrospective analysis.According to different treatment methods,all the patients were divided into a western medicine group(n=89)and a combination group(n=103).Patients in the western medicine group received western medicine treatment,while patients in the combination group received conventional western medicine combined with modified Kushen decoction treatment.The course of treatment in both groups was 4 weeks.The clinical efficacy,skin lesion degree[eczema area and severity index(EASI)scores],pruritus degree[visual analogue scale(VAS)score],peripheral blood inflammatory factors and adverse conditions were compared between the two groups.Results The total effective rate of clinical treatment in combined group was 97.1%,which was higher than 88.8% in western group(P<0.05).After treatment,the EASI scores of lesion area,papule,exfoliation and lichenization in combined group were lower than those in western medicine group(P<0.05).After 2 and 4 weeks of treatment,VAS scores in combined group were lower compared to western medicine group(P<0.05).After treatment,the levels of γ-interferon,interleukin-4 and C-reactive protein(CRP)in combined group were lower than those in western medicine group(P<0.05).There was no statistical difference in the total incidence rate of adverse drug reactions between the combined group(6.8%)and the western group(3.4%)(P>0.05).Conclusion Modified Kushen decoction combined with western medicine has a significant clinical effect in the treatment of patients with chronic eczema of spleen deficiency and dampness accumulation type.It can effectively alleviate the skin lesions and pruritus symptoms,and improve the inflammatory indicators,and it has good safety.
