肿瘤患者输液港使用期间无损伤针置换时间的应用研究

摘要 目的 探讨肿瘤患者完全植入式静脉输液港(totally implantable venous access devices,TIVAD)使用期间无损伤针的置换时间。方法 采用便利抽样法于2021年1月-2022年12月在浙江大学医学院附属第一医院纳入植入TIVAD的肿瘤患者106例,采用随机数字表法分为对照组和观察组各53例。对照组常规7 d置换无损伤针,观察组14d置换无损伤针,比较两组患者拔管后感染发生情况。结果 研究期间,两组置管期间感染发生率差异无统计学意义(P>0.05);但观察组TIVAD留置时间长于对照组、感染患者植入TIVAD至感染发生时间晚于对照组,差异有统计学意义(P<0.05)。结论 适当延长肿瘤患者TIVAD使用期间无损伤针更换时间不会增加其感染发生风险,还可在一定程度上延缓感染的发生,并延长TIVAD的留置时间。 Objective To explore the replacement time of non-invasive needles during the use of total implantable venous access devices(TIVAD)for tumor patients.Methods One hundred and six tumor patients implanted with TIVAD were enrolled in the First Affiliated Hospital of Zhejiang University School of Medicine from January 2021 to December 2022 by convenience sampling method.They were divided into a control group and an observation group based on random number table method,with 53 patients in each group.The control group received routine 7-day replacement of non-invasive needles,while the observation group received 14-day replacement of non-invasive needles.Compare the incidence of infection after extubation between two groups of patients.Results During the study period,there was no statistically significant difference in the incidence of infection between the two groups during catheterization(P>0.05),However,the observation group had longer TIVAD retention time than the control group,and infected patients implanted TIVAD until the infection occurred later than the control group(P<0.05).Conclusion Properly extending the time for non-destructive needle replacement during the use of TIVAD in tumor patients will not increase their risk of infection,but can also delay the occurrence of infection to a certain extent and extend the retention time of TIVAD.
