肾缺血再灌注损伤对大鼠肾小球滤过屏障超微结构的影响

摘要 本研究探讨了大鼠肾缺血再灌注损伤(IRI)后肾小球滤过屏障(GFB)的超微结构变化。12只SD大鼠随机均分为假手术组(Sham组)和缺血再灌注损伤组(IRI组),分别采用常规透射电镜技术和硝酸镧示踪技术观察GFB的超微结构变化。Sham组电镜下可见GFB三层结构完整,有孔内皮细胞紧贴于基底膜(GBM)内侧,GBM厚度均匀,足细胞足突呈栅栏状紧贴在GBM外侧,足突间裂孔隔膜清晰可见;IRI组电镜下可见广泛性足突融合,裂孔消失,融合的足突间封闭连接取代了裂孔隔膜,足细胞表面出现大量微绒毛,局部GBM不规则增厚,并出现分层和断裂,足细胞足突从GBM脱离,造成GBM外侧裸露,导致GFB结构不完整。Sham组硝酸镧示踪法电镜下可见大量镧盐颗粒沉积于内皮窗孔和GBM内疏层,少见镧盐颗粒穿过GBM致密层和外疏层进入到肾小囊腔;IRI组硝酸镧示踪法电镜下可见大量镧盐沉积于内皮窗孔和GBM各层,并有部分镧盐颗粒进入肾小囊腔。研究结果表明肾IRI可破坏GFB的完整性,导致GFB通透性显著增加。 This study investigated the ultrastructural changes in the glomerular filtration barrier(GFB)induced by renal ischemia-reperfusion injury(IRI)in rats.Twelve male SD rats were randomly divided into two groups:The Sham group(Sham group)and the Ischemia-reperfusion injury group(IRI group)with six rats in each group.Ultrastructural changes in the GFB were examined by transmission electron microscopy and a lanthanum nitrate tracer technique.In the Sham group,the structure of the GFB was normal,including the fenestrated endothelium,the intervening glomerular basement membrane(GBM),and the podocyte slit diaphragm.The fenestrated endothelium sticked closely to the inner side of the intervening GBM.The podocyte foot processes were arranged in a palisading pattern,adhering to the outside of the GBM.The slit diaphragm between the foot processes was clearly visible.In the IRI group,extensive effacement of the foot processes was observed.The slit diaphragm between foot processes disappeared,replaced by occluding junctions.Many microvilli were observed on the surface of podocytes.Partial GBM was irregular and thickened,displaying separated layer and rupture.The podocyte foot processes detached from the GBM,leaving parts of the GBM exposed and compromising the integrity of the GFB.Lanthanum nitrate tracer showed that many lanthanite particles deposited in the fenestra of endothelial cell and the lamina rarainterna of the GBM in the Sham group,with few lanthanite particles crossing the lamina densa and lamina raraexterna of GBM into Bowman’s space.In the IRI group,many lanthanite particles deposited in the endothelial cell fenestra and three layers of the GBM,with some lanthanite particles entering Bowman’s space.These findings showed that IRI disrupted the integrity of the GFB and increased its permeability.
