摘要
目的通过数据挖掘探讨针刺治疗卒中后睡眠障碍国内选穴规律。方法检索建库至2024年6月中国知网、万方数据知识服务平台、维普网针刺治疗卒中后睡眠障碍临床研究文献,建立数据库并进行频次分析、关联规则、聚类分析并建立复杂网络图。结果共纳入文献123篇,提取针刺处方169首,腧穴累计应用1848次;针刺治疗卒中后睡眠障碍以督脉腧穴应用频次最多,足太阳膀胱经、足阳明胃经其次,应用频次最高的腧穴为神门、百会、三阴交;关联规则显示,针刺治疗卒中后睡眠障碍使用频率最高的穴对为“内关-三阴交”“内关-百会”“百会-神门”;聚类分析可得6组核心聚类,其中神门与百会自成一类,为针刺治疗卒中后睡眠障碍的重点要穴;复杂网络图显示,核心腧穴为百会、安眠、神门、三阴交。结论针刺治疗卒中后睡眠障碍选穴以通阳达脑,调血安神为原则,临床多以神门、百会、三阴交为核心穴组,取督脉、足太阳膀胱经、足阳明胃经进行施治。
Objective To explore the domestic acupoint selection rule of acupuncture in the treatment of post-stroke sleep disorders through data mining.Methods Clinical research literature on acupuncture treatment of post-stroke sleep disorders from CNKI,Wanfang Data,and VIP was retrieved from the inception to June 2024.The database was established and frequency analysis,association rules,cluster analysis,and complex network diagram were conducted.Results A total of 123 articles were included,169 acupuncture prescriptions were extracted,and the acupoints were applied for 1848 times.The most frequently used acupoints for acupuncture treatment of post-stroke sleep disorders were governor vessel acupoints,followed by bladder meridian of foot taiyang and stomach meridian of foot yangming.The most frequently used acupoints were shénmén,bǎihuì,and sānyīnjiāo.The association rules showed that the most frequently used acupoint pairs for acupuncture treatment of post-stroke sleep disorders were“nèiguān-sānyīnjiāo”,“nèiguān-bǎihuì”,and“bǎihuì-shénmén”.Cluster analysis could obtain six groups of core clusters,among which shénmén and bǎihuìwere of their own category,which were the key points for acupuncture treatment of post-stroke sleep disorders.The complex network diagram showed that the core acupoints were bǎihuì,ānmi?觃n,shénmén,and sānyīnjiāo.Conclusion The principle of acupuncture treatment of post-stroke sleep disorder is to dredge yang and reach the brain,regulate blood and tranquilize the mind.Clinically,shénmén,bǎihuì,and sānyīnjiāo are the core acupoint groups,and the governor vessel,bladder meridian of foot taiyang and stomach meridian of foot yangming are selected for treatment.
