摘要
复发性流产是妇产科常见妊娠并发症之一,病因错综复杂,甚至半数以上患者病因不明,称为不明原因复发性流产。其相关治疗方案受到广大学者关注。中医药治疗本病有独特性优势。邓阿黎教授在长期临床实践中总结出“两期三循八法”治疗不明原因复发性流产,即从“孕前和孕后”两期着手,遵循“预培其损、治病安胎、中西合参”三大原则,恪守孕前“清、培、调、疏”与孕后“活、健、凉、和”八大治法,能有效改善不明原因复发性流产患者不良症状和降低流产率。现归纳总结邓阿黎教授治疗不明原因复发性流产临床经验,以飨同道。
Recurrent miscarriage is one of the common pregnancy complications in obstetrics and gynaecology,with complex etiology,even more than half of the patients have unknown etiology,which is called unexplained recurrent miscarriage.Its related therapeutic options have attracted the attention of scholars.Traditional Chinese medicine(TCM)has unique characteristics and advantages in treating this disease.Professor Deng Ali summed up the“two-phase,three-principles,eight-phase”treatment of unexplained recurrent miscarriage in his long-term clinical practice,starting from the“pre-pregnancy and post-pregnancy”phases,following the three major principles of“pre-cultivation of its loss,treatment of the disease to stabilise the foetus and combination of Chinese and Western medicine”.In other words,starting from the“pre-pregnancy”and“post-pregnancy”periods,following the three major principles of“pre-cultivation of its damage,treatment of disease and foetal safety,and combination of Chinese and Western medicines”,and adhering to the eight major treatments of“clearing,cultivating,regulating,and sparing”in pre-pregnancy periodand“activating,strengthening,cooling,and harmonising”in post-pregnancy period,the treatment is able to improve the adverse symptoms of patients with recurrent miscarriages of unknown origin and reduce the rate of miscarriage effectively.The clinical experience of Professor Deng in treating unexplained recurrent miscarriage is summarised as follows for the benefit of his colleagues.

王莹(1999-),女,湖北中医药大学硕士研究生,研究方向为中医药治疗妇科常见病;通讯作者:邓阿黎(1974-),女,,教授,主任医师,医学博士,博士研究生导师,湖北省中医院妇产科主任、湖北省中医院健康生殖中心副主任,师从全国名中医姜惠中教授,是全国名老中医药专家姜惠中学术思想继承人、全国中医药管理局姜惠中传承工作室核心成员,兼任湖北省中医(中西医结合)妇产专科联盟秘书长、世界中医药学会联合会妇科专业委员会理事、湖北省中医师协会生殖医学专业委员会副主任委员、湖北省中医药学会妇科专业委员会常委、湖北省中医药学会内分泌专业委员会常委、全国中医临床特色技术传承骨干人才,长期从事中医妇产科临床、科研和教学工作,擅长运用中西医结合治疗不孕症、多囊卵巢综合征、复发性流产、子宫内膜异位症、月经病等妇科常见疾病及疑难病症,尤其对复发性流产的治疗颇有心得。任职以来,共培养14名研究生与1名博士生。承担了中医妇科学、西医妇产科学的本专科生的课堂教育及本专科生、研究生、进修生的临床带教工作,并获得2013年度、2014年度“优秀教师”称号。在2020年抗击新冠肺炎疫情防控工作中勇于奉献,获得湖北中医药大学的“抗疫先锋”称号,2020年度获得湖北省中医院“优秀医师”“最美医生”称号。2021年获得湖北中医药大学“医德标兵”称号。2013年至首都医科大学附属复兴医院宫腔镜诊疗中心“宫腹腔镜技术”进修学习。2020年参与全国中医临床特色技术传承骨干人才培训项目。主持及参与国家“十五”“十一五”科技攻关项目、973计划、国家科技支撑计划、省科技厅、省卫健委、省教育厅、武汉市晨光计划、武汉市科技攻关等十余个项目,如:补肾活血中药方对复发性流产小鼠Tre g/Th17细胞因子影响(湖北省自然科学基金项目),姜惠中治疗多囊卵巢综合征临床经验传承研究(湖北省卫健委面上项目),补肾化痰方调控AMPK/mT0R/ULK1信号通路改善自噬活性影响PMOP骨/脂代谢平衡的机制研究(国家自然科学基金项目)等。在研项目有:加味寿胎丸调控NLRP3/Caspase-1/IL-1β信号通路介导巨噬细胞焦亡治疗不明原因复发性流产的疗效及机制研究(湖北省自然科学基金项目),mTOR/HIF-1α/ERRα通路调控有氧糖酵解代谢重编程影响骨形成及补肾化痰方的干预机制研究(国家自然科学基金项目)。此外,在核心期刊上发表论文50余篇,出版论著6部。研究方向为中医药治疗妇科常见病。E-mail:1556635835@qq.com。
