摘要
目的比较化学发光免疫技术和放射免疫技术在甲状腺肿瘤诊断中的价值。方法回顾性选取2022年5月至2024年5月徐州医科大学附属医院收治的110例疑似甲状腺肿瘤患者。比较两种不同检测手段检测出的生化指标水平、甲状腺肿瘤检测情况以及诊断效能情况。结果采用化学发光免疫技术测定游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)、促甲状腺素(TSH)、甲状腺球蛋白(Tg)的水平均高于放射免疫技术测定的结果(P<0.001)。110例疑似甲状腺肿瘤患者中,病理检查结果显示阳性88例,阴性22例;化学发光免疫法检测显示阳性86例,阴性24例,放射免疫法检测出阳性82例,阴性28例。化学发光免疫法诊断甲状腺肿瘤的灵敏度为95.45%、特异度为90.91%、准确度为94.55%,显著高于放射免疫法对甲状腺肿瘤诊断的灵敏度(79.55%)、特异度(45.45%)和准确度(72.73%),差异有统计学意义(P<0.05)。结论在甲状腺肿瘤患者的诊断中,化学发光免疫技术具有较高的灵敏度、特异度和准确度。
Objective To compare the value of chemiluminescence immunoassay(CLIA)and radioimmunoassay(RIA)in the diagnosis of thyroid tumors.Methods A total of 110 patients with suspected thyroid tumors,treated at the Affiliated Hospital of Xuzhou Medical University from May 2022 to May 2024,were selected for the study.The biochemical indicator levels,thyroid tumor detection results,and diagnostic performance of the two different methods were compared.Results The levels of free triiodothyronine(FT3)(P<0.001),free thyroxine(FT4)(P<0.001),thyroid-stimulating hormone(TSH)(P<0.001),and thyroglobulin(P<0.001)measured by CLIA were significantly higher than those measured by RIA.Among the 110 patients with suspected thyroid tumors,88 cases were pathologically confirmed as positive and 22 as negative.CLIA detected 86 positive cases and 24 negative cases,while RIA detected 82 positive cases and 28 negative cases.The sensitivity,specificity,and accuracy of CLIA for diagnosing thyroid tumors were 95.45%,90.91%,and 94.55%,respectively,which were significantly higher than those of RIA,with a sensitivity of 79.55%,specificity of 45.45%,and accuracy of 72.73%(P<0.05).Conclusion Chemiluminescence immunoassay demonstrates higher sensitivity,specificity,and accuracy in diagnosing thyroid tumors compared to radioimmunoassay.
