摘要
目的:探讨健脾益肾化瘀汤联合醋酸泼尼松片对脾肾两虚夹瘀型肾病综合征患儿肾功能、炎症指标和中医症状的影响。方法:选取信阳市中心医院收治的符合纳入标准的120例脾肾两虚夹瘀型肾病综合征患儿,依据随机数字表随机分为观察组和对照组,每组各60例。对照组在常规治疗基础上给予醋酸泼尼松片治疗,观察组在对照组治疗基础上给予健脾益肾化瘀汤进行治疗,两组均连续治疗12周。比较两组患儿治疗前后肢体倦怠、面色无华、夜尿频多等中医症状评分,肾功能(SCr、UACR、BUN、CCR),炎症指标(IL-6、hs-CRP)的变化情况,同时比较临床疗效和不良反应发生率。结果:观察组总有效率93.33%(56/60),显著高于对照组总有效率75%(45/60),两组比较差异有统计学意义(P<0.01)。与治疗后比较,两组肢体倦怠、面色无华、夜尿频多等中医症状评分均明显降低(P<0.05);与对照组治疗后比较,观察组肢体倦怠、面色无华、夜尿频多等中医症状评分均明显降低(P<0.01)。与治疗后比较,两组UACR、BUN、SCr水平均明显降低(P<0.05),CCR水平明显升高(P<0.05);与对照组治疗后比较,观察组UACR、BUN、SCr水平均明显降低(P<0.01),CCR水平明显升高(P<0.01)。与治疗后比较,两组患儿IL-6、hs-CRP水平明显降低(P<0.05);与对照组治疗后比较,观察组IL-6、hs-CRP水平均明显降低(P<0.01)。观察组不良反应发生率6.67%(4/60),对照组不良反应发生率11.67%(7/60),两组比较差异无统计学意义(P>0.05)。结论:健脾益肾化瘀汤联合醋酸泼尼松片治疗儿童脾肾两虚夹瘀型肾病综合征疗效显著,能够改善肾功能和炎症反应,减少不良反应的发生。
Objective:To investigate the effects of Jianpi Yishen Huayu Decoction combined with prednisone acetate tablets on renal function,inflammatory markers,and traditional Chinese medicine(TCM)symptoms in children with nephrotic syndrome of dual deficiency of spleen and kidney and blood stasis type.Methods:A total of 120 children with nephrotic syndrome of dual deficiency of spleen and kidney and blood stasis type,treated at Xinyang Central Hospital,were randomly divided into an observation group and a control group,with 60 cases in each group.The control group was treated with prednisone acetate tablets in addition to conventional treatment,while the observation group received the same plus Jianpi Yishen Huayu Decoction.Both groups were treated for 12 consecutive weeks.TCM symptom scores(including fatigue,pale complexion,and frequent nocturnal urination),renal function indicators(SCr,UACR,BUN,CCR),and inflammatory markers(IL-6,hs-CRP)were compared before and after treatment.Clinical efficacy and adverse reaction rates were also compared.Results:The total effective rate in the observation group was 93.33%(56/60),significantly higher than the 75%(45/60)in the control group(P<0.01).After treatment,TCM symptom scores(fatigue,pale complexion,frequent nocturnal urination)in both groups significantly decreased(P<0.05),and the reduction was more significant in the observation group(P<0.01).After treatment,UACR,BUN,and SCr levels significantly decreased,and CCR levels significantly increased in both groups(P<0.05),with more pronounced changes in the observation group(P<0.01).Similarly,IL-6 and hs-CRP levels were significantly reduced in both groups(P<0.05),with more significant reductions in the observation group(P<0.01).The incidence of adverse reactions was 6.67%(4/60)in the observation group and 11.67%(7/60)in the control group,with no statistically significant difference between the two groups(P>0.05).Conclusion:Jianpi Yishen Huayu Decoction combined with prednisone acetate tablets shows significant therapeutic efficacy in treating children with nephrotic syndrome of dual deficiency of spleen and kidney and blood stasis type.It can improve renal function and reduce inflammatory responses,with fewer adverse reactions.
