摘要
目的 探讨血清微小RNA-432(miR-432)、血管内皮生长因子(VEGF)、糖类抗原125(CA125)联合检测在肺结核患者中的诊断价值。方法 选取2021年4月至2023年6月漯河市传染病医院收治的70例肺结核患者为病例组,另选取70例同期健康体检者为对照组;依据肺结核患者临床症状体征,分为活动期肺结核患者及结核患者。对比两组入院时血清miR-432、VEGF、CA125水平及C反应蛋白(CRP)、肿瘤坏死因子-a(TNF-a)水平,并分析其相关性;对比不同病程分型患者入院时血清各指标水平,并分析入院时各指标联合检测对肺结核的诊断价值。结果病例组入院时血清miR-432、VEGF、CA125、CRP、TNF-a水平均高于对照组(P<0.05);入院时病例组血清miR-432、VEGF、CA125与CRP、TNF-a均呈正相关(P<0.05);活动期肺结核患者入院时血清miR-432、VEGF、CA125水平均高于潜伏期肺结核患者(P<0.05);入院时血清miR-432、VEGF、CA125水平联合诊断肺结核的曲线下面积(AUC)为0.937。结论 血清miR-432、VEGF、CA125联合检测可提高肺结核的诊断效能,对临床诊断有辅助作用。
Objective To investigate the diagnostic value of serum micro RNA-432(miR-432),vascular endothelial growth factor(VEGF) and carbohydrate antigen 125(CA125) combined detection in patients with pulmonary tuberculosis.Methods A total of 70 patients with pulmonary tuberculosis admitted to Luohe infectious disease hospital from April 2021 to June 2023 were selected as the study group,and 70 healthy people during the same period were selected as the control group.Patients were divided into active stage pulmonary tuberculosis and latent stage pulmonary tuberculosis group according to the clinical symptoms.MiR-432,VEGF,CA125,C-reactive protein(CRP) and tumor necrosis factor-A(TNF-a) were compared between the two groups on admission,and their correlation was analyzed.Serum indexes level of patients with different stage were compared on admission,and the diagnostic value of combined detection of all indexes on admission was analyzed.Results The levels of miR-432,VEGF,CA125,CRP and TNF-a in the study group were higher than those in the control group(P<0.05).MiR-432,VEGF and CA125 were positively correlated with CRP and TNF-a in the study group on admission(P<0.05).The levels of miR-432,VEGF and CA125 in patients with active pulmonary tuberculosis were higher than those with latent pulmonary tuberculosis infection(P<0.05).The area under the curve( AUC) of serum miR-432,VEGF and CA125 combined detection was 0.937 on admission.Conclusion The combined detection of serum miR-432,VEGF and CA125 can improve the diagnostic efficiency of pulmonary tuberculosis and play an auxiliary role in clinical diagnosis.
