摘要
目的探讨肺结核与肺癌之间的关系、发病危险因素,以及临床特征和诊治,为肺结核合并肺癌的早期发现提供临床参考。方法通过光镜及免疫组织化学染色分析2例肺结核合并肺癌,并结合相关文献进行复习讨论。结果2例患者抗酸染色(+)、肺组织HE染色均见肿瘤细胞,瘤细胞体积较大,胞浆丰富、嗜酸性,核圆形、卵圆形,核大深染;免疫组化示CK-pan、CK5/6、TTF-1、Ki-67均为阳性。结论肺结核和肺癌关系密切,两者相互影响。肺结核会增加肺癌的发病风险,其发病机制主要与慢性炎症、免疫功能异常、药物因素、瘢痕组织相关。癌症本身所具有的免疫调节作用及抗癌治疗也可能会增加肺结核的发生风险。快速明确诊断及早期治疗是提高患者生存率及预后的有效手段。
Objective To explore the relationship between pulmonary tuberculosis and lung cancer,risk factors,clinical characteristics,diagnosis and treatment,and to provide clinical reference for the early detection of pulmonary tuberculosis complicated with lung cancer.Methods 2 cases of pulmonary tuberculosis complicated with lung cancer were analyzed by light microscopy and immunohistochemical staining,and the relevant literatures were reviewed and discussed.Results Acid fast staining(+)and lung tissue HE staining showed tumor cells in both patients.The tumor cells were large in volume,rich in cytoplasm,eosinophilic,round nuclei,oval nuclei and large deep staining.Immunohistochemistry showed that CK-pan,CK5/6,TTF-1 and Ki-67 were positive.Conclusion Pulmonary tuberculosis and lung cancer are closely related and influence each other.Pulmonary tuberculosis can increase the risk of developing lung cancer,and its pathogenesis is mainly related to chronic inflammation,immune dysfunction,drug factors and scar tissue.The immunomodulatory effects of cancer itself and anti-cancer treatment may also increase the risk of developing TB.Rapid and clear diagnosis and early treatment are effective means to improve the survival rate and prognosis of patients.
