摘要
目的比较甲苯磺酸瑞马唑仑和丙泊酚用于门诊老年患者拔牙的麻醉效果及满意度。方法选取北京口腔医院2022年11月至2023年3月于门诊无痛中心行复杂牙拔除术的老年患者66例,采取随机数字表法分成甲苯磺酸瑞马唑仑组(T组)和丙泊酚组(C组),每组33例。T组给予甲苯磺酸瑞马唑仑0.04 mg/kg iv。C组给予丙泊酚0.3 mg/kg iv。比较两组首次用药前5 min(T0)、首次用药后5 min(T1)、手术开始后5 min(T2)、手术结束时(T3)、手术结束后10 min(T4)、离院时(T5)的收缩压(SBP)、舒张压(DBP)、心率(HR)、呼吸频率(RR)、血氧饱和度、镇静评分、VAS镇痛评分、患者满意评分、医生满意评分及不良反应等指标。结果两组患者在T1时的收缩压、舒张压、心率和呼吸频率均有明显下降,与各时间点相比,T1时T组的SBP、DBP下降平缓,差异有统计学意义(P<0.05)。与各时间点相比,T2时两组患者SBP、DBP和RR均有所回升,T组的SBP、DBP均高于C组,差异有统计学意义(P<0.05)。T组的医生满意度评分和患者满意度评分均值在9.5分以上,均高于C组,医生满意度评分和患者满意度评分与C组比较差异有统计学意义(P<0.05)。结论甲苯磺酸瑞马唑仑应用于门诊老年患者拔牙时,与丙泊酚相比,具有镇静、抗焦虑、血流动力学波动小、安全性更高等诸多优点,医生及患者满意度更高,具有较好的临床应用价值。
Objective To compare the anesthetic effect and satisfaction of remazolam tosylate and propofol for tooth extraction in geriatric outpatient.Methods A total of 66 elderly patients who underwent complex tooth extraction in the outpatient painless center of Beijing Stomatological Hospital from November 2022 to March 2023 were selected and divided into ramazolam tosylate group(group T)and propofol group(group C)by random number table method,with 33 cases in each group.Group T was given ramazolam tosylate 0.04 mg/kg iv.Group C was given propofol 0.3 mg/kg iv.Systolic blood pressure(SBP),diastolic blood pressure(DBP),heart rate(HR),respiratory rate(RR),blood oxygen saturation,sedation score,VAS analgesia score,patient satisfaction score and doctor's satisfaction score were compared between the two groups 5 minutes before first medication(T0),5 minutes after first medication(T1),5 minutes after surgery(T2),at the end of surgery(T3),10 minutes after surgery(T4),and at discharge from hospital(T5)Meaning score and adverse reactions and other indicators.Results SBP,DBP,HR and RR in both groups decreased significantly at T1,and SBP and DBP in T group at T1 decreased gently compared with each time point,with statistical significance(P<0.05).Compared with each time point,SBP,DBP and RR in both groups increased at T2,and SBP and DBP in group T were higher than those in group C,with statistical significance(P<0.05).The average scores of doctors'satisfaction and patients'satisfaction in group T were above 9.5 points,both higher than those in group C,and the differences between doctors'satisfaction and patients'satisfaction were statistically significant(P<0.05).Conclusions Compared with propofol,remazolam tosylate has many advantages such as sedation,anti-anxiety,low hemodynamic fluctuation,higher safety and higher satisfaction of doctors and patients,so it has better clinical application value.
