四磨汤加味方联合多潘立酮对老年胃癌根治术后胃肠功能紊乱的影响

Effects of Modified Simo Decoction Combined with Domperidone on Gastrointestinal Dysfunction after Radical Gastrectomy in Elderly Patients with Gastric Cancer

摘要目的:探讨四磨汤加味方联合多潘立酮对老年胃癌根治术后胃肠功能紊乱的影响。方法:纳入2021年1月—2023年5月于长治市人民医院就诊的胃癌根治术后胃肠功能紊乱患者80例,按随机数表方式分为观察组和对照组,每组40例。对照组给予多潘立酮治疗,观察组在对照组基础上加用四磨汤加味方。比较两组临床疗效,同时评估其中医证候积分以及胃肠功能恢复情况,另外评估两组胃激素指标[胃动素(MTL)、胃泌素(GAS)、血管活性肽肠(VIP)],比较两组不良反应发生情况。结果:观察组、对照组总有效率分别为95.00%(38/40)、80.00%(32/40),观察组高于对照组(P<0.05);两组治疗后各中医证候积分较治疗前降低(P<0.01),且观察组低于对照组(P<0.01);观察组胃肠功能恢复时间短于对照组(P<0.01);两组治疗后MTL、GAS升高而VIP降低(P<0.01),观察组治疗后MTL、GAS高于对照组(P<0.01),VIP低于对照组(P<0.01);两组不良反应总发生率差异无统计学意义(P>0.05)。结论:四磨汤加味方联合多潘立酮用于治疗老年胃癌根治术后胃肠道紊乱疗效显著,不仅能有效改善患者临床症状、体征,还能调节其胃肠道激素水平,具备安全性。 Objective:To investigate the effects of Modified Simo Decoction combined with domperidone on gastrointestinal dysfunction in elderly patients after radical gastrectomy for gastric cancer.Methods:A total of 80 patients with gastrointestinal dysfunction after radical gastrectomy for gastric cancer,who were treated at Changzhi People's Hospital from January 2021 to May 2023,were included in the study.The patients were randomly divided into an observation group and a control group,with 40 cases in each group.The control group received domperidone treatment,while the observation group received Modified Simo Decoction in addition to the treatment given to the control group.The clinical efficacy,Traditional Chinese Medicine(TCM)syndrome scores,and recovery of gastrointestinal function were compared between the two groups.Additionally,the levels of gastrointestinal hormones[motilin(MTL),gastrin(GAS),and vasoactive intestinal peptide(VIP)]and the incidence of adverse reactions were evaluated.Results:The total effective rate was 95.00%(38/40)in the observation group and 80.00%(32/40)in the control group,with the observation group showing significantly better results(P<0.05).Post-treatment,TCM syndrome scores decreased in both groups compared to pre-treatment(P<0.01),with lower scores in the observation group(P<0.01).The recovery time of gastrointestinal function was shorter in the observation group(P<0.01).After treatment,MTL and GAS levels increased,while VIP levels decreased in both groups(P<0.01);the observation group had higher MTL and GAS levels and lower VIP levels compared to the control group(P<0.01).There was no statistically significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion:Modified Simo Decoction combined with domperidone is effective in treating gastrointestinal dysfunction after radical gastrectomy in elderly gastric cancer patients.It not only significantly improves clinical symptoms and signs but also regulates gastric hormone levels,with a good safety profile.
