Effects of different anesthesia schemes on anesthetic effect and postoperative cognitive function in elderly patients undergoing percutaneous nephrolithotomy

摘要目的:分析不同麻醉方案对老年经皮肾镜碎石取石术患者麻醉效果及术后认知功能的影响。方法:以2021年12月1日—2023年12月31日在连州市人民医院接受经皮肾镜碎石取石术的90例老年患者为研究对象,采用计算机随机分配法分为对照组和治疗组,每组各45例。对照组在全身麻醉下进行手术,治疗组在硬膜外阻滞下进行手术,对比两组的麻醉效果及术后认知功能。结果:两组麻醉效果优良率对比,差异无统计学意义(P>0.05)。麻醉前及麻醉3 d后,两组简易智能精神状态检查量表(MMSE)评分对比,差异无统计学意义(P>0.05);麻醉后6 h、1 d及2 d,治疗组MMSE评分均较对照组高(P<0.05)。结论:对老年经皮肾镜碎石取石术患者实施硬膜外阻滞的麻醉效果与全身麻醉基本一致,但可显著减少麻醉药物的使用剂量,对患者术后认知功能的影响较小,有较高的临床应用及推广价值。 Objective To analyze the effects of different anesthesia schemes on anesthetic effect and postoperative cognitive function in elderly patients undergoing percutaneous nephrolithotomy.Methods A total of 90 elderly patients who received percutaneous nephrolithotomy in Lianzhou People's Hospital from December 1,2021 to December 31,2023 were selected as the research objects,and were divided into control group and treatment group by computer random allocation method,with 45 cases in each group.The control group received surgery under general anesthesia,and the treatment group received surgery under epidural block.The anesthetic effect and postoperative cognitive function of the two groups were compared.Results There was no significant difference in the excellent and good rate of anesthetic effect between the two groups(P>0.05);Before anesthesia and 3 days after anesthesia,there was no significant difference in the score of Mini-Mental State Examination(MMSE)between the two groups(P>0.05);The MMSE score of the treatment group was higher than that of the control group at 6 h,1 days and 2 days after anesthesia(P<0.05).Conclusion The anesthetic effect of epidural block for elderly patients undergoing percutaneous nephrolithotomy is basically the same as that of general anesthesia,but the dose of anesthetic drugs can be significantly reduced,and the effect on postoperative cognitive function of patients is small,which has high clinical application and promotion value.
