摘要
目的 探讨微创经皮钢板内固定(MIPPO)技术联合锁定加压钢板治疗胫骨下段骨折的效果。方法 将120例胫骨下段骨折患者根据手术方式不同分为对照组(采用常规切开复位内固定治疗,60例)和观察组(采用MIPPO技术联合锁定加压钢板治疗,60例)。记录两组手术情况、骨折愈合情况、关节活动度、临床治愈率及并发症发生情况,采用膝关节Rasmussen评分、踝关节Baird-Jackson评分评价疗效。结果 患者均获得3个月随访。术中出血量、住院时间观察组少(短)于对照组(P<0.05)。手术时间、骨痂形成时间两组比较差异均无统计学意义(P>0.05)。术后3个月膝关节Rasmussen评分、踝关节Baird-Jackson评分、踝关节活动度、临床治愈率观察组均优于对照组(P<0.05)。并发症发生率对照组高于观察组(P<0.05)。结论 MIPPO技术联合锁定加压钢板治疗胫骨下段骨折可以减少术中出血量,缩短住院时间,减少术后并发症的发生,促进关节功能的恢复。
Objective To investigate the effect of minimally invasive percutaneous plate osteosynthesis(MIPPO)technique combined with locking compression plate in the treatment of distal tibial fracture.Methods The 120 patients with lower tibial fractures were divided into control group(60 cases were treated by conventional open reduction and internal fixation)and observation group(60 cases were treated by MIPPO technique combined with locking compression plate),according to different surgical methods.The operation situation,fracture healing,joint mobility,clinical cure rate and complications of the two groups were recorded.The curative effect was evaluated by r asmussen score of knee joint and Baird-Jackson score of ankle joint.Results All patients were followed up for 3 months.The intraoperative blood loss and hospitalization time in the observation group were less(shorter)than those in the control group(P<0.05).There were no statistical differences in operation time and callus formation time between the two groups(P>0.05).Three months after operation,r asmussen score of knee joint,Baird-Jackson score of ankle joint,range of motion of ankle joint and clinical cure rate of observation group were better than those of control group(P<0.05).The incidence rate of complications in the control group was higher than that in the observation group(P<0.05).Conclusions MIPPO technique combined with locking compression plate in the treatment of lower tibial fracture can reduce intraoperative blood loss,shorten hospital stay,reduce postoperative complications and promote joint function recovery.
