摘要
目的探讨外周血中性粒细胞/淋巴细胞比值(NLR)联合C反应蛋白(CRP)/前白蛋白(PA)比值在成人甲型流感患者早期的诊断价值。方法回顾性分析2022年1月至2024年1月在本院经甲流抗原诊断试剂(胶体金)检测的甲型流感阳性患者34例,为甲流组。以甲流抗原阴性患者32例为对照组。经全自动血细胞分析仪和全自动生化分析仪检测得出NLR和CRP/PA比值。分析2组NLR和CRP/PA比值的差异性。采用受试者工作特征(ROC)曲线预测NLR联合CRP/PA比值在成人甲流患者早期预测诊断的价值。结果对照组NLR均值为(3.42±1.85)、CRP/PA比值均值为(0.03±0.03);甲流组NLR均值为(6.23±3.10),CRP/PA比值均值为(0.15±0.13);甲流组患者NLR和CRP/PA比值均高于对照组患者,差异均有统计学意义(t=4.485、5.120,P均<0.05)。NLR诊断甲型流感的ROC曲线面积(95%CI)为0.776(0.666,0.888),截断值为5.12,灵敏度为0.65,特异度为0.84。CRP/PA比值诊断甲型流感的曲线面积(95%CI)为0.874(0.794,0.954),截断值为0.04,灵敏度为0.94,特异度为0.66。NLR联合CRP/PA比值诊断甲流的ROC曲线面积(95%CI)为0.928(0.862,0.994),灵敏度为0.82,特异度为0.99。结论NLR和CRP/PA比值均可以作为独立预测甲型流感的生物标志物。NLR与CRP/PA比值联合检测在成人甲型流感早期预测诊断效能更大,为甲流患者早期治疗起到积极作用。
Objective To investigate the diagnostic value of peripheral blood neutrophil/lymphocyte ratio(NLR)and CRP/PA ratio in the early stage of adult influenza A patients.Methods Thirty-four patients tested for influenza A positive antigen diagnostic reagent(colloidal gold)from January 2022 to January 2024.Thirty-two patients negative for all H 1 x antigen were used as control group.The NLR and CRP/PA ratio were detected by the fully automatic blood cell analyzer and the fully automatic biochemical analyzer.The variability of the NLR and CRP/PA ratios between the two groups was analyzed.The receiver operating characteristic(ROC)curve was used to predict the value of NLR and CRP/PA ratio in early prediction diagnosis.Results The mean NLR of the control group was(3.42±1.85).The mean value of and CRP/PA ratio is(0.03±0.03);the mean NLR of the group was(6.23±3.10),and the mean of CRP/PA ratio was(0.15±0.13);the NLR and CRP/PA ratio in the first flu group were higher than the control group,significant(t=4.485,5.120,P<0.05).The ROC curve area(95%CI)for NLR diagnosis of influenza A was 0.776(0.666,0.888)with a cutoff of 5.12,sensitivity of 0.65 and specificity of 0.84.The curve area(95%CI)of the CRP/PA ratio to diagnose influenza A was 0.874(0.794,0.954),with a cutoff of 0.04,a sensitivity of 0.94,and a specificity of 0.66.The ROC curve area(95%CI)for NLR combined with CRP/PA ratio was 0.928(0.862,0.994),sensitivity was 0.82 and specificity 0.99.Conclusion Both NLR and CRP/PA ratio can be used as biomarkers for independent prediction of influenza A.The combined detection of NLR and CRP/PA ratio is greater in the early prediction and diagnosis of influenza A in adults,and plays a positive role in the early treatment of influenza a flu patients.
