摘要
目的了解老年慢性阻塞性肺疾病(COPD)稳定期患者营养不良的现状并分析其影响因素。方法采用便利抽样法选取2022年1月至2023年3月在金华市中心医院门诊复诊的84例COPD稳定期患者作为研究对象,采用一般情况调查表、精简微型营养评价量表(MNA-SF)、美国心血管健康研究(CHS)衰弱评估表、6分钟步行试验(6MWT)、COPD评估测试(CAT)及呼吸困难指数(mMRC)对其进行调查。结果84例老年COPD稳定期患者营养不良风险发生率为83.33%,营养不良与衰弱呈正相关(P<0.01)。多因素logistic回归分析结果显示,不同体重指数(BMI)和衰弱是老年COPD稳定期患者营养不良风险的影响因素(P<0.05)。结论老年COPD稳定期患者营养不良风险发生率较高。医护人员及家庭照护者应加以重视,结合影响因素制定针对性干预方案,以降低老年COPD稳定期患者营养不良风险。
Objective To understand the current status of malnutrition in elderly patients with chronic obstructive pulmonary disease(COPD)in the stable stage and analyze its influencing factors.Methods Eighty-four patients with stable COPD who were rechecked at the outpatient clinic of the Jinhua Central Hospital from January 2022 to March 2023 were selected by convenience sampling method as study subjects,and were assessed using a general information questionnaire,the mini nutritional assessment short-form(MNA-SF),the Cardiovascular Health Study(CHS)frailty assessment scale,the 6-minute walk test(6MWT),COPD assessment test(CAT)and modified medical research council(mMRC)were investigated.Results The prevalence of malnutrition risk in 84 elderly patients with stable COPD was 83.33%.Malnutrition was positively correlated with frailty(P<0.01),and different body mass indexes(BMI)and frailty were influential factors in the risk of malnutrition in elderly patients with stable COPD(P<0.05),as shown by multiple linear regression analysis.Conclusion Elderly patients with stable COPD have a high prevalence of malnutrition risk.Healthcare professionals and family caregivers should pay attention to this issue and develop targeted intervention programs that combine the influencing factors to reduce the risk of malnutrition in elderly patients with stable COPD.
