摘要
目的:探讨在老年2型糖尿病合并骨折患者围术期中应用精细化管理的效果。方法:选取2021年11月-2023年3月重庆市铜梁区中医院收治的100例老年2型糖尿病合并骨折患者作为研究对象,随机分为试验组和对照组,每组50例。对照组实施常规护理,试验组实施精细化管理。比较两组护理满意度、血糖水平、用药依从性、骨折术后指标。结果:试验组护理满意度、Morisky用药依从性问卷评分高于对照组(P<0.05)。护理后,两组空腹血糖、餐后2 h血糖、糖化血红蛋白低于护理前,且试验组低于对照组(P<0.05)。试验组骨折愈合时间、下床活动时间、住院时间短于对照组(P<0.001)。结论:精细化管理在老年2型糖尿病合并骨折患者围术期中的应用效果较好,能够控制血糖水平,促进骨折愈合,缩短患者住院时间,提高其用药依从性和护理满意度。
Objective:To investigate the effect of fine management in elderly patients with type 2 diabetes complicated with fracture during perioperative period.Methods:A total of one hundred elderly patients with type 2 diabetes mellitus combined with fracture treated in Chongqing Tongliang District Hospital of Traditional Chinese Medicine from November 2021 to March 2023 were selected as the study objects and randomly divided into experimental group and control group,with fifty cases in each group.The control group was given routine nursing,and the experimental group was given fine management.Nursing satisfaction,blood glucose level,medication compliance and postoperative fracture indexes were compared between the two groups.Results:The scores of nursing satisfaction and Morisky medication compliance in experimental group were higher than those in control group(P<0.05);after nursing,fasting blood glucose,2 h postprandial blood glucose and HBA1c in both groups were lower than before nursing,and the experimental group was lower than the control group(P<0.05);the time of fracture healing,getting out of bed and staying in hospital in experimental group were shorter than those in control group(P<0.001).Conclusion:The application of fine management in the perioperative period of elderly patients with type 2 diabetes mellitus complicated with fracture has a good effect,which can control blood glucose level,promote fracture healing,shorten the hospital stay of patients,and improve their medication compliance and nursing satisfaction.
