摘要
目的探索临床药师在社区开展居家药学服务对提高老年慢病患者合理用药水平的影响.方法选取2023年1-6月的社区老年慢病患者,随机分为对照组和实验组,通过设计问卷调查分析老年慢病患者不合理用药的可能原因,临床药师结合互联网药学服务,针对性地给予实验组线上线下相结合的居家药学服务,对照组给予常规社区随访管理,比较干预前后患者用药依从性、临床指标控制情况、ADR发生率.结果临床药师给予居家药学服务干预后,实验组患者用药依从性、临床指标控制均高于对照组,ADR的发生率实验组低于对照组.结论临床药师开展居家药学服务可以明显提高老年慢病患者合理用药水平.
Objective To explore the clinical pharmacists to carry out home pharmaceutical care in the community and improve the level of rational drug use in elderly patients with chronic diseases.Methods Community elderly patients with chronic diseases from January to June 2023 were selected and randomly divided into control group and experimental group.The possible causes of irrational drug use of elderly patients with chronic diseases were analyzed through a questionnaire survey designed.Clinical pharmacists combined with Internet pharmaceutical care provided targeted online and offline in-home pharmaceutical care to the experimental group,and routine community follow-up management was given to the control group.The medication compliance,control of clinical indicators and incidence of ADR were compared before and after the intervention.Results After clinical pharmacists gave home pharmaceutical care intervention,the medication compliance and clinical index control of patients in the experimental group were higher than those in the control group,and the incidence of ADR in the experimental group was lower than that in the control group.Conclusion Home pharmaceutical care by clinical pharmacists can significantly improve the level of rational drug use in elderly patients with chronic diseases.
