体型特异性剂量估算在学龄儿童心脏CTA中的应用

摘要 目的比较容积CT剂量指数(CTDIvol)与体型特异性剂量估算(SSDE)两种测量方法测得的辐射剂量在学龄儿童心脏CTA检查中的差异并探讨其原因。方法回顾性分析我院77例学龄儿童心脏CTA检查的影像资料,手工勾勒中心层面轮廓作为感兴趣区(ROI),测量ROI的平均最大左右径(LAT)、CT值(CT_(ROI))、面积(A_(ROI))。依据LAT进行分组:A组LAT<22cm,33例;B组22cm≤LAT<25cm,26例;C组25cm≤LAT<30cm,18例。记录并计算转换因子F_(DW)和SSDE_(Dw),分析CTDI_(vol)与SSDE_(Dw)的差异,利用多元回归分析性别、年龄、AROI和LAT与SSDEDW的相关性。结果A、B、C组SSDEDw较CTDIvol分别高197.37%、192.01%、187.68%,各组间CTDIvol与SSDEDw均存在差异(P<0.05)。LAT与SSDEDw存在明显正相关关系(P<0.05),性别、年龄、AROI与SSDEDw无明显相关关系(P>0.05)。结论SSDE_(Dw)相比CTDI_(vol)评估学龄儿童心脏CTA辐射剂量更为准确。LAT与SSDE_(Dw)呈正线性相关关系,LAT每增加1cm,SSDE_(Dw)增加0.44MGy。 Objective To explore the differences in radiation doses in School-age children by comparing volume CT dose index(CTDIvol)and body size-specific dose estimates(SSDE)in cardiac CTA examinations,and to explore the reasons.Methods A retrospective analysis was conducted on the imaging data of 77 children's cardiac CTA examinations in our hospital.Manual outlining of the center layer contour as the region of interest(ROI)was performed,and the mean maximum lateral diameter(LAT),average CT value(CTROI),and area(AROI)of the ROI were measured.According to LAT,the subjects were divided into three groups:Group A(LAT<22cm,n=33);Group B(22cm≤LAT<25cm,n=26);Group C(25cm≤LAT<30cm,n=18).The conversion factors FDW and SSDEDw were calculated and recorded.The differences between CTDIvol and SSDEDw were analyzed,and the correlation between gender,age,AROI,and LAT with SSDEDw was studied using multiple regression analysis.Results The SSDEDw values for Groups A,B,and C were 197.37%,192.01%,and 187.68%higher than those of CTDIvol.There were significant differences between CTDIvol and SSDEDw among the three groups(P<0.05).A positive linear relationship was observed between LAT and SSDEDw(P<0.05).Gender,age,and AROI showed no significant correlation with SSDEDw(P>0.05).Conclusion Compared with CTDIvol,SSDEDw is more accurate in assessing radiation dose in children's cardiac CTA.There is a positive linear relationship between LAT and SSDEDw,whereby LAT increases by 1 cm,SSDEDw increases by 0.44MGy.
