摘要
目的探讨前哨淋巴结活检与保乳术联合用于乳腺癌治疗的效果。方法回顾性研究2018年1月—2023年12月福建医科大学附属第二医院接治的80例原发性乳腺癌患者的临床资料。基于手术方式不同分成研究组(前哨淋巴结活检^(+)保乳术)与对照组(改良根治术),各40例。对比2组手术情况、预后及机体免疫功能及术后并发症发生率。结果研究组的手术用时、拔管时间、住院时间、术中出血量、引流量均低于对照组(P<0.05)。术前,2组患者CD3^(+)、CD4^(+)/CD8^(+)水平比较,差异无统计学意义(P>0.05);术后CD3^(+)、CD4^(+)/CD8^(+)均有降低,但研究组均高于对照组(P<0.05)。术前2组生活质量评分比较,差异无统计学意义(P>0.05);术后,研究组生活质量评分高于对照组(P<0.05)。研究组的术后并发症发生率为7.5%,对照组为20.0%,差异无统计学意义(P>0.05)。结论前哨淋巴结活检联合保乳术用于乳腺癌治疗有确切效果,术后恢复快,并发症少,预后良好。
Objective To investigate the effect of sentinel lymph node biopsy combined with breast-conserving surgery in the treatment of breast cancer.Methods A retrospective study was conducted on the clinical data of 80 patients with primary breast cancer admitted to the Second Affiliated Hospital of Fujian Medical University from January 2018 to December 2023.Based on different surgical methods,they were divided into a study group(sentinel lymph node biopsy+breast-conserving surgery)and a control group(modified radical mastectomy),with 40 cases in each group.The surgical conditions,prognosis,immune function and incidence of postoperative complications were compared between the two groups.Results The surgical time,extubation time,hospitalization time,intraoperative blood loss,and drainage volume of the study group were all lower than those of the control group(P<0.05).Before surgery,there was no statistically significant+difference in the levels of CD3 and CD4^(+)/CD8^(+)between the two+groups of patients(P>0.05).After surgery,both CD3 and CD4^(+)/CD8^(+) decreased,but the study group was higher than the control group(P<0.05).There was no statistically significant difference in the quality of life scores between the two groups before surgery(P>0.05).After surgery,the quality of life score of the study group was significantly higher than that of the control group(P<0.05).The incidence of postoperative complications was 7.5%in the study group and 20.0%in the control group,the difference was not statistically significant(P>0.05).Conclusion Sentinel lymph node biopsy combined with breast-conserving surgery has a definite effect in the treatment of breast cancer,with rapid postoperative recovery,fewer complications and a good prognosis.
