摘要
目的调查结直肠癌患者家庭照顾者的决策疲劳现状并分析其影响因素。方法2023年4月至2024年1月,采取方便抽样方法,抽取山东第一医科大学附属省立医院肿瘤科、胃肠外科、中医科病房住院的149例结直肠癌患者及其平均每天照顾时间最长的1名家庭照顾者为研究对象。采用多点横断面研究设计,应用自行设计的一般情况调查表、中文版决策疲劳量表(DFS)、照顾者积极感受量表(PAC)、领悟社会支持量表(PSSS)及简易应对方式问卷进行面对面调查。采用多重线性逐步回归分析家庭照顾者决策疲劳的影响因素。结果结直肠癌患者家庭照顾者的DFS评分为(9.19±2.52)分。患者性别、年龄和医疗费用支付方式及照顾者年龄、工作状态、文化程度、与患者关系、婚姻状况和平均每天照顾时长均影响结直肠癌患者家庭照顾者DFS评分(P<0.05)。PAC、PSSS及其各维度和积极应对方式与DFS均呈负相关(P<0.01),消极应对方式与DFS呈正相关(P<0.01)。影响结直肠癌患者家庭照顾者的决策疲劳因素依次为照顾者平均每天照顾时长、PSSS、照顾者年龄和消极应对方式(P<0.05),共解释总变异的73.5%。结论结直肠癌患者家庭照顾者的决策疲劳处于轻度水平,且影响因素的水平不一,针对影响水平较高的因素,积极进行适当干预,有助于降低家庭照顾者的决策疲劳水平。
Objective To investigate decision fatigue status in family caregivers of patients with colorectal cancer, and analyze influencing factors. Methods From April 2023 to January 2024, a convenience sampling method was used to select 149 patients with colorectal cancer admitted to Department of Oncology, Department of Gastrointestinal Surgery, and Department of Traditional Chinese Medicine Wards of Affiliated Provincial Hospital of Shandong First Medical University, as well as the family caregiver with the longest average daily care time as the research subjects. A multi-point cross-sectional study design was adopted, and a self-designed general situation questionnaire, decision fatigue scale (DFS), positive aspects of caregiving (PAC), perceived social support scale (PSSS), and simplified coping style questionnaire were used for face-to-face surveys. Multiple linear stepwise regression was used to analyze influencing factors of decision fatigue among family caregivers. Results DFS score of family caregivers for colorectal cancer patients was (9.19±2.52) points. The patient’s gender, age and payment method for medical expenses, as well as the caregiver’s age, work status, educational level, relationship with the patient, marital status, and average daily care time affected DFS score of family caregivers of colorectal cancer patients (P<0.05). PAC, PSSS, and their dimensions, as well as positive coping styles were negatively correlated with DFS (P<0.01), while negative coping style was positively correlated with DFS (P<0.01). The influencing factors of decision fatigue among family caregivers of colorectal cancer patients were average daily care time for caregivers, PSSS, caregiver age, and negative coping styles (P<0.05), accounting for 73.5% of total variation. Conclusion Decision fatigue among family caregivers of colorectal cancer patients is at a mild level, and the level of influencing factors varies. For the factors with high influence levels, active, and appropriate intervention is helpful to reduce the level of decision fatigue of family caregivers.
