摘要
目的探讨老年帕金森病患者衰弱的危险因素。方法选取2014年9月—2024年2月在四川大学华西医院住院的147例老年帕金森病患者,根据FRAIL评分结果将患者分为衰弱和非衰弱,采用单因素分析和二元logistic回归分析,探讨老年帕金森病患者衰弱发生的危险因素。结果147例老年帕金森病患者中,有58例(39.4%)患者存在衰弱。衰弱、非衰弱老年帕金森病患者年龄、BMI、D-二聚体、血小板、白蛋白、Barthel评分、疼痛数字评分(NRS)、华西跌倒评分比较差异有统计学意义(P均<0.05)。二元logistic回归分析显示:BMI(OR=0.777,95%CI:0.679~0.889)、白蛋白(OR=0.773,95%CI:0.773~0.933)、Barthel评分(OR=0.981,95%CI:0.967~0.995)、NRS(OR=1.529,95%CI:1.132~2.067)是老年帕金森病患者衰弱发生的影响因素(P均<0.05)。结论老年帕金森病患者衰弱发生率相对较高,低BMI、低白蛋白、低Barthel评分、高NRS是老年帕金森病患者衰弱发生的危险因素。
Objective To identify the risk factors associated with frailty in elderly individuals diagnosed with Parkinson's disease.Methods A total of 147 elderly Parkinson's disease patients who were hospitalized in West China Hospital of Sichuan University between September 2014 and February 2024 were selected.Based on their FRAIL score results,participants were categorized into frail and non-frail groups.Univariate analysis and binary logistic regression were employed to examine the risk factors for frailty in this population.Results Among of the 147 patients,58(39.4%)were identified as frail.Significant differences were observed in age,body mass index(BMI),D-dimer levels,platelet count,albumin levels,Barthel score,NRS,and Huaxi Fall Assessment scale between the frail and non-frail groups(all P<0.05).The binary logistic regression analysis indicated that BMI(OR=0.777,95%CI:0.679-0.889),albumin levels(OR=0.773,95%CI:0.773-0.933),Barthel score(OR=0.981,95%CI:0.967-0.995),and NRS(OR=1.529,95%CI:1.132-2.067)were significant factors influencing frailty in elderly Parkinson's patients(all P<0.05).Conclusions The prevalence of frailty among elderly patients with Parkinson's disease is notably high.Factors such as low BMI,low albumin levels,low Barthel scores,and high NRS are identified as risk factors for frailty in this demographic.
