孕期体脂百分数对妊娠期糖尿病及胎儿不良结局的影响

摘要 目的:探讨孕期体脂百分数对妊娠期糖尿病(Gestational diabetes mellitus,GDM)发生风险及胎儿不良结局的影响。方法:分析2021年1月至2023年1月医院常规产检的1200例产妇的资料,根据孕妇体脂百分数分为低体脂组(n=480)、普通组(n=420)、高体脂组(n=420)。妊娠24-28 w,采用口服葡萄糖耐糖实验(Oral Glucose Tolerance Test,OGTT)测量孕妇空腹(Fasting blood glucose,FBG)、口服葡萄糖1 h(1-hour blood glucose,1hPBG)、口服葡萄糖2 h(2-hour blood glucose,2hPBG)的血糖值;记录分娩情况,将胎儿结局分为不良结局组和良好结局组,比较两组产妇的基线资料及入组时体脂百分数。分析体脂百分数对胎儿不良结局的影响。结果:高体脂组血糖水平、GDM发生率高于低体脂组、普通组(P<0.05)。1200例孕妇中不良结局发生率为3.0%。不良结局组体脂百分数、血糖水平高于结局良好组(P<0.05)。Logistic回归分析显示,血糖水平、体脂百分数是胎儿不良结局的危险因素(OR>1,P<0.05)。结论:孕期体脂百分数的异常会增加孕妇发生GDM的风险,且会增加胎儿不良结局风险。 Objective:To investigate the effects of body fat percentage during pregnancy on the risk of gestational diabetes mellitus(GDM)and fetal adverse outcomes.Methods:Data from 1200 pregnant women who underwent routine prenatal care in the hospital from January 2021 to January 2023 were analyzed.According to the percentage of body fat in pregnant women,they were divided into the low body fat group(n=480),the ordinary group(n=420),and the high body fat group(n=420).During the 24-28 weeks of pregnancy,the pregnant women were tested for fasting blood glucose(FBG),1-hour blood glucose(1hPBG),and 2-hour blood glucose(2hPBG)using an oral glucose tolerance test(OGTT).The delivery conditions were recorded,and the fetal outcomes were divided into an adverse outcome group and a good outcome group.The baseline data and body fat percentage at enrollment were compared between the two groups of pregnant women.The effect of body fat percentage on adverse fetal outcomes was analyzed.Results:The blood glucose level and incidence of GDM in the high body fat group were higher than those in the low body fat group and the general group(P<0.05).The incidence of adverse outcomes among 1200 pregnant women was 3.0%.The body fat percentage and blood glucose level in the adverse outcome group were higher than those in the good outcome group(P<0.05).Logistic regression analysis showed that blood glucose level and body fat percentage were risk factors for adverse fetal outcomes(OR>1,P<0.05).Conclusion:Abnormal body fat percentage during pregnancy increases the risk of GDM in pregnant women and increases the risk of adverse fetal outcomes.
