基因芯片技术快速检测结核分枝杆菌耐药基因的应用价值研究 被引量:1

Evaluation of Gene Chips for the Rapid Diagnosis of Drug-Resistance Genes in Mycobacterium Tuberculosis

摘要 目的:探讨基因芯片技术在结核分枝杆菌快速鉴定及耐药分析中的应用价值。方法:收集我院2022年10月—2023年10月收治疑似结核病患者的临床样本,包括痰液、肺泡灌洗液、脓液或分泌物、穿刺液(含胸腹水和脑脊液)等,同时进行MGIT960接种培养、药敏试验和基因芯片直接检测法,对两种方法的检测效能进行比对分析。结果:(1)以MGIT960培养鉴定为参考标准,基因芯片法直接检测临床样本中结核分枝杆菌的灵敏度为83.33%,特异度为92.86%,一致性为88.46%,Kappa值为0.766。(2)以MGIT960药敏试验为参考标准,基因芯片法检测直接检测临床样本中结核分枝杆菌利福平耐药的灵敏度为81.82%,特异度为94.87%,一致性为92.00%,Kappa值为0.767;检测异烟肼耐药的灵敏度为75.00%,特异度为92.86%,一致性为90.00%,Kappa值为0.755。结论:以MGIT960培养法及药敏试验为参考标准,基因芯片技术直接检测临床原始标本的灵敏度、特异度高,有较好的一致性。应用基因芯片直接检测临床样本有助于结核病的早期诊断及治疗。 Objective:To explore the application value of gene chip technology in rapid identification of Mycobacterium tuberculosis and drug resistance analysis.Methods:Collect the same clinical sample of suspected tuberculosis patients admitted to our hospital from October 2022 to October 2023,including sputum,alveolar lavage fluid,pus or secretion,puncture fluid(including hydrothorax,ascites,and cerebrospinal fluid),etc.Conducted MGIT960 inoculation culture,drug sensitivity test and gene chip direct detection method to compare and analyze the detection efficiency of the two methods.Results:(1)Using MGIT960 culture identification as the reference standard,the sensitivity of gene chip method for detecting Mycobacterium tuberculosis was 83.33%,specificity was 92.86%,consistency was 88.46%,and Kappa value was 0.766,respectively.(2)Using MGIT960 drug sensitivity test as the reference standard,the sensitivity of gene chip method for detecting rifampicin resistance in Mycobacterium tuberculosis was 81.82%,specificity was 94.87%,consistency was 92.00%,and Kappa value was 0.767,respectively;the sensitivity of detecting isoniazid resistance in Mycobacterium tuberculosis was 75.00%,specificity was 92.86%,consistency was 90.00%,and Kappa value was 0.755,respectively.Conclusion:Using MGIT960 culture method and drug sensitivity test as reference standards,gene chip technology directly detects clinical original specimens with high sensitivity,specificity,and good consistency.Direct detection of original clinical specimens is helpful to the early diagnosis,prevention and control of tuberculosis.
