摘要
目的:以能力、机会、动机-行为模型(COM-B模型)为理论指导,探讨慢性心力衰竭(CHF)病人多重用药管理的影响因素,为临床制定干预措施提供依据。方法:2023年11月—2024年1月采用目的抽样法选取18例CHF病人进行半结构式访谈,采用定向内容分析法进行资料分析。结果:共提炼出3个主题和8个亚主题,即能力因素(疾病及药物知识、药物管理技能)、机会因素(治疗负担、医疗支持、家庭支持)和动机因素(服药信念、主观能动性、负性情绪)。结论:CHF病人多重用药管理行为受多种因素影响,医护人员应采取针对性措施提高病人的药物管理能力,加强多元社会支持,培养积极的药物管理动机,进而改善病人服药行为。
Objective:To explore the influencing factors of polypharmacy management in patients with chronic heart failure(CHF)based on the capability,opportunity,and motivation-behavior model(COM-B model),and provide a basis for primary health care institutions to formulate clinical intervention measures.Methods:A semi-structured interview was conducted with a total of 18 patients with CHF from November 2023 to January 2024 by using purposive sampling method,and the data were analyzed by directed content analysis.Results:A total of 3 themes and 8 sub-themes were extracted,namely,ability factors(disease and medication knowledge,medication management skills),opportunity factors(treatment burden,medical support,family support)and motivation factors(belief in taking medicine,subjective initiative,negative emotions).Conclusions:The polypharmacy management behavior of CHF patients is affected by many factors.The medical staff should take targeted measures to improve the medication management ability of patients,strengthen the multi-social support,and cultivate the positive medication management motivation,so as to improve the medicine-taking behavior of patients.
