摘要
目的分析梅毒螺旋体抗体检测(TPAB)联合快速血浆反应素环状卡片试验(RPR)在梅毒血清学检测中的临床价值。方法选定本院2021年1月至2022年12月门诊就诊的150例高度疑似梅毒患者,分别给予TPAB、RPR检查,将梅毒螺旋体明胶颗粒凝集试验(TPPA)作为诊断金标准,比较TPAB、RPR、TPAB+RPR诊断准确率、灵敏度、特异度,并进行Kappa一致性检验结果分析。结果TPPA检测阳性、阴性者分别116例、34例;TPAB+RPR诊断准确率(97.3%)、灵敏度(99.1%)均高于TPAB(90.7%、88.0%)、RPR(91.4%、88.8%)(P<0.05),TPAB+RPR诊断特异度(91.2%)与TPAB(88.2%)、RPR(85.3%)比较差异无统计学意义(P>0.05);TPAB与TPPA对比一致性一般,Kappa值为0.603;RPR与TPPA对比无一致性,Kappa值为0.297;TPAB+RPR与TPPA对比一致性较好,Kappa值为0.933。结论TPAB、RPR联合可提高梅毒诊断准确率、灵敏度,具有重要的参考价值。
Objective To analyze the clinical value of treponema pallidum antibody test(TPAB)combined with rapid plasma reagin ring card test(RPR)in serological detection of syphilis.Methods A total of 150 patients with high suspected syphilis who received outpatient treatment in our hospital from January 2021 to December 2022 were selected and given TPAB and RPR examinations respectively.TPPA was used as the diagnostic gold standard,and the diagnostic accuracy,sensitivity and specificity of TPAB,RPR and TPAB+RPR were compared.The results of Kappa consistency test were analyzed.Results One hundred and sixteen cases of TPPA were positive,34 cases were negative.The diagnostic accuracy(97.3%)and sensitivity(99.1%)of TPAB+RPR were higher than those of TPAB(90.7%、88.0%)and RPR(91.4%、88.8%)(P<0.05).The diagnostic specificity of TPAB+RPR(91.2%)was compared with that of TPAB(88.2%)and RPR(85.3%)(P>0.05).The comparison between TPAB and TPPA was generally consistent,and Kappa value was 0.603.There was no consistency between RPR and TPPA,and the Kappa value was 0.297.TPAB+RPR was consistent with TPPA,and the Kappa value was 0.933.Conclusion The combination of TPAB and RPR can improve the diagnostic accuracy and sensitivity of syphilis,and has important reference value.
