多部门协作在细菌耐药监测工作中的应用及效果

Application and effect of multi-department collaboration in monitoring bacterial resistance

摘要 目的探讨多部门协作管理模式,延缓细菌耐药发生。方法成立由药学部、医院感染管理科、医务部、微生物室、临床科室、护理部、信息科多部门组成的监管小组,修订相关规章制度、明确部门职责、细化考核指标,各部门协作进行细菌耐药知识培训、追踪检查、召开协作小组会议等。结果提高了抗菌药物使用合理率、细菌耐药监测能力及其预防控制措施的执行力,抗菌药物使用率由2020年上半年的39.9%降至下半年的39.7%;多重耐药菌医院感染例次率由2020年上半年的0.6%降至下半年的0.2%;耐碳青霉烯类细菌检出率由2020年上半年的5.5%降至下半年的4.2%。结论采取多部门协作管理模式、运用PDCA循环法、建立长效管理机制,可以提高抗菌药物使用合理率、降低MDRO感染例次率,有效遏制细菌耐药。 Objective To explore the multi-department cooperative management mode to delay the occurrence bacterial resistance.Methods Establishing a regulatory group consisting of multiple departments including the medicine department,hospital infection management section,medical department,microbial room,nursing department and clinical departments,information section,to revise relevant rules and regulations,clear the responsibilities,detailed assessment indicators,collaborate with various departments to provide training on bacterial resistance knowledge,track and inspect,and hold collaborative team meetings,etc.Results The rational rate of antibiotics utilization,the monitoring of bacterialresistance and the implementation of prevention and control measures were improved.The utilization rate of antibiotics decreased from 39.9% in the first half of 2020 to 39.7% in the second half of 2020.The nosocomial infection rate of multidrug-resistant bacteria decreased from 0.6% in the first half of 2020 to 0.2% in the second half of 2020.The detection rate of carbapenem-resistant bacteria decreased from 5.5% in the first half of 2020 to 4.2% in the second half of 2020.Conclusion Adopting the multi-department cooperative management mode,utilizing the PDCA cycle method and establishing a long-term management mechanism can improve the rational rate of antibiotic utilization,reduce the case rate of multi-drug resistant Organism(MDRO)infection,and effectively curb bacterial resistance.
