摘要
目的探讨预防性放置钛夹对内镜下治疗结直肠息肉迟发性出血的影响。方法选择海南医学院第一附属医院于2022年1月至2023年10月收治的120例结直肠息肉患者作为研究对象,均接受内镜黏膜切除术治疗,根据组间基线资料匹配的原则,采用随机数字表法分为观察组(n=60)与对照组(n=60)。观察组在术中预防性放置钛夹止血,对照组则不放置。比较两组患者术中止血、术后恢复及迟发性出血等指标。结果两组患者的即时止血率、操作时间比较,差异无统计学意义(P>0.05);观察组患者有效止血率明显高于对照组,术中止血治疗时间明显短于对照组比较,差异有统计学意义(P<0.05);与对照组比较,观察组患者潜血转阴时间及住院时间明显更短,差异有统计学意义(P<0.05);肠鸣音恢复时间在两组比较,差异无统计学意义(P>0.05);观察组患者术后迟发性出血发生率及需内镜下止血率明显低于对照组,差异有统计学意义(P<0.05);两组患者的需输血比例差异无统计学意义(P>0.05)。结论内镜下治疗结直肠息肉术中预防性放置钛夹对可以更加有效的止血,对预防迟发性出血的发生发挥着重要作用。
Objective To investigate the effect of prophylactic placement of titanium clips on endoscopic treatment of delayed bleeding in patients with colorectal polyps.Method 120 patients with colorectal polyps treated in The First Affiliated Hospital of Hainan Medical College from January 2022 to October 2023 were studied,all of whom underwent endoscopic mucosal resection,and were randomly divided into two groups,the observation group(n=60)and control group(n=60)according to the random number table method,The observation group received prophylactic placement of titanium clips for hemostasis during surgery,while the control group.The indicators of intraoperative hemostasis,postoperative recovery,and delayed bleeding in both groups were compared.Results For immediate hemostasis rate and operation time,had no significant difference between two groups(P>0.05).In the observation group compared with the control group,the effective hemostasis rate was higher significantly,and the intraoperative hemostasis treatment time was shorter significantly(P<0.05);the observation group had a significantly shorter time for occult blood to turn negative and hospital stay compared the control group,(P<0.05);Between the two groups,there was of no significant difference in the recovery time of bowel sounds(P>0.05).In the observation group,The incidence of postoperative delayed bleeding and the need for endoscopic hemostasis compared with the control group were significantly lower(P<0.05),in blood transfusion rate in both groups had no significant difference(P>0.05).Conclusion During endoscopic treatment of colorectal polyps,the prophylactic placement of titanium clips was more effective in stopping bleeding and had a better preventive effect on the occurrence of delayed bleeding.
