摘要
甲状腺结节是甲状腺最常见的一种疾病,属于中医学“瘿病”范畴。早期甲状腺结节,可无明显症状,随着病情发展可出现颈部包块、颈部压迫感、声音嘶哑等症状,部分甲状腺结节还具有癌变风险。目前西医治疗方法多以随访、手术、消融为主,存在病情延误、长期服药等不足,因此在临床中探讨和总结甲状腺结节的治疗经验与思路具有重要意义。倪英群老师认为痰瘀既是甲状腺结节的主要病理因素也是其发生发展的主要病因病机,治疗时应以“痰瘀”为切入点,分期辨证论治:早期益气健脾,兼理气化痰消瘀,防病邪深入;中期以理气化痰为主,兼活血消瘀,以防病情加重;晚期以化痰消瘀散结为主,兼顾护正气。该文从病因病机、治则治法、用药心得、验案举隅等方面总结了倪师治疗甲状腺结节的临床经验,以供同道参考。
Thyroid nodule is one of the most common diseases of thyroid,which belongs to the category of"gall disease"in traditional Chinese medicine.Early thyroid nodules,may have no obvious symptoms,with the development of the disease may appear neck mass,neck pressure,hoarseness and other symptoms,some thyroid nodules also have the risk of cancer.At present,Western medicine mainly focuses on follow-up,surgery and ablation,and there are deficiencies such as delayed disease and long-term medication.Therefore,it is of great significance to discuss and summarize the experience and ideas of thyroid nodule treatment in clinical practice.treatment of thyroid nodules.Teacher Ni Yingqun believed that phlegm stasis was not only the main pathological factor of thyroid nodule but also the main cause and pathogenesis of its occurrence and development.The treatment should be based on"phlegm stasis"as the starting point,and stage differentiation and treatment:early tonifying qi and strengthening spleen,regulating qi and eliminating phlegm and eliminating blood stasis,preventing disease and pathogenic factors from deepening;In the middle stage,it is mainly to regulate qi and eliminate phlegm,simultaneously promoting blood circulation and eliminating blood stasis to prevent the aggravation of the disease;In the late stage,the main purpose is to eliminate phlegm and dissipate stasis,taking into account the protection of healthy qi.In this paper,Ni Yingqun's clinical experience in treating thyroid nodules was summarized from the aspects of etiology,pathogenesis,treatment method,medication experience and case study.
