摘要
脓毒性休克是临床危急重症,因其高发病率、高死亡率、救治难度大,成为重症医学面临的难题及挑战。而中医药治疗脓毒症有多靶点、多环节、多系统调节宿主的炎症风暴及器官功能障碍的优势。文章总结了郑进教授治疗脓毒性休克经验,郑进教授认为本病病机为正气虚于内,外感毒气,湿、热、毒、瘀相互内结,伤脏损络,最终导致气血逆乱,严重者多脏器受损,阴阳离决而出现死亡。治疗重在调和气血、顾护心肺。同时,结合疾病进展所处的不同阶段,分别采取宣透、截断扭转、扶正祛毒等方法。根据不同的证型推荐临床常用方剂,拟定治疗思路,取得了较为满意的疗效,可为本病的临床治疗提供借鉴。
Septic shock is a clinical critical disease.Because of its high incidence rate,high mortality and difficult treatment,it has become a difficult problem and challenge for severe medicine.Traditional Chinese medicine has the advantage of multiple targets,links,and systems to regulate the host's inflammatory storm and organ dysfunction in treating sepsis.The article summarizes Professor Zheng Jin's experience in treating septic shock.Professor Zheng Jin believes that the pathogenesis of this disease is the deficiency of vital energy in the internal environment,with external toxic gas,dampness,heat,toxin,and blood stasis intertwined,causing damage to the organs and collaterals,ultimately leading to the disorder of qi and blood.In severe cases,multiple organs are damaged,and Yin and Yang are isolated,resulting in death.The key to treatment is to regulate qi and blood,and take care of the heart and lungs.At the same time,taking into account the different stages of disease progression,methods such as promoting transparency,cutting and twisting,and strengthening the body to eliminate toxins are adopted.Recommend commonly used clinical formulas based on different syndrome types and develop treatment strategies.Satisfactory therapeutic effects have been achieved,which can provide reference for the clinical treatment of this disease.
