摘要
目的:围绕肝硬化门静脉高压患者,在经颈静脉肝内门体静脉分流术(TIPS)治疗后,采用彩色多普勒超声(简称“彩超”)检查,评定其在监测TIPS治疗效果中的价值。方法:对在本院择期行TIPS的60例肝硬化门静脉高压男性患者进行选取,实施彩超检查,测定门静脉内径(PVD)、门静脉血流速度(PVV)、脾静脉内径(SVD)、脾静脉血流速度(SVV)、肠系膜上静脉内径(SMVD)、肠系膜上静脉血流速度(SMVV)及脾脏厚度等指标。将术中所测定的门静脉压力梯度(PPG)当作金标准,对比手术前、后各指标差值,并探讨各指标与PPG间的相关性。结果:TIPS术后的PPG较术前低(P<0.05)。与TIPS术前相比,术后的PVD、SMVD、SVD与之相比,无明显差异(P>0.05),而SMVV、SVV、PVV均有增加(P<0.05),脾脏厚度有显著减小(P<0.05)。SVV、PVV、SMVV与PPG之间呈现明显的负相关(P<0.05),而SMVD、PVD、脾脏厚度、SVD与其没有相关性(P>0.05)。结论:彩超指标SMVV、PVV与SVV能够将TIPS手术前、后门静脉压力情况反映出来,故可当作监测TIPS术后短期疗效的一种重要手段。
Objective:After treatment with transjugular intrahepatic portosystemic shunt(TIPS),color Doppler ultrasound(CDUS)was used to evaluate the value of monitoring the therapeutic effect of TIPS in patients with liver cirrhosis and portal hypertension.Methods:60 male patients with cirrhosis and portal hypertension who underwent TIPS at our hospital were selected and underwent color ultrasound examination to measure portal vein diameter(PVD),portal vein flow velocity(PVV),splenic vein diameter(SVD),splenic vein flow velocity(SVV),superior mesenteric vein diameter(SMVD),superior mesenteric vein flow velocity(SMVV),and splenic thickness.Using the portal vein pressure gradient(PPG)measured during surgery as the gold standard,compare the differences between various indicators before and after surgery,and explore the correlation between each indicator and PPG.Results:The PPG after TIPS surgery was lower than before(P<0.05).Compared with before TIPS surgery,there was no significant difference in postoperative PVD,SMVD,and SVD(P>0.05),while SMVV,SVV,and PVV all increased significantly(P<0.05),and spleen thickness decreased significantly(P<0.05).There was a significant negative correlation(P<0.05)between SVV,PVV,SMVV and PPG,while there was no correlation(P>0.05)between SMVD,PVD,spleen thickness,and SVD.Conclusion:Color Doppler ultrasound indicators SMVV,PVV,and SVV can reflect the pre-and post-operative portal vein pressure of TIPS,and can be used as an important means to monitor the short-term efficacy of TIPS surgery.
