经颅彩色多普勒超声参数与肺超声评分的相关性及对新生儿呼吸窘迫综合征的诊断价值

Correlations between transcranial color Doppler ultrasound parameters and lung ultrasound score and the diagnostic value for neonatal respiratory distress syndrome

摘要 目的 探究经颅彩色多普勒超声参数与肺超声评分的相关性及对新生儿呼吸窘迫综合征的诊断价值。方法 收集2020年3月—2022年3月温州市人民医院收治的98例疑似新生儿呼吸窘迫综合征患儿,进行经颅彩色多普勒超声检查,检查后确诊为呼吸窘迫综合征的49例患儿为研究组,非呼吸窘迫综合征的49例新生儿为对照组。比较两组患儿经颅彩色多普勒超声参数,比较不同严重程度的研究组患儿经颅彩色多普勒超声参数,比较不同严重程度的研究组患儿肺超声评分,分析经颅彩色多普勒超声参数、肺超声评分与疾病严重程度的关系,分析经颅彩色多普勒超声参数与肺超声评分的相关性,分析经颅彩色多普勒超声参数对呼吸窘迫综合征的诊断意义。结果 对照组收缩期峰值血流速度(PSV)、平均血流速度(MFV)、舒张末期血流速度(EDV)、搏动指数(PI)、阻力指数(RI)分别为(43.26±8.69)cm/s、(14.53±4.58)cm/s、(25.75±6.84)cm/s、(0.57±0.09)、(0.89±0.31);研究组PSV、MFV、EDV、PI、RI分别为(33.61±6.37)cm/s、(9.07±3.46)cm/s、(18.11±5.82)cm/s、(0.71±0.11)、(1.39±0.45)。两组PSV、MFV、EDV、PI、RI比较差异均有统计学意义(t=6.269,6.658,5.955,6.895,6.405,均P<0.05)。非危重组、危重组、急危重组患儿PSV、MFV、EDV、PI、RI比较,差异均有统计学意义(均P<0.05)。非危重组、危重组、急危重组肺超声评分分别为(14.56±5.93)分、(18.11±6.04)分、(21.57±6.49)分,差异有统计学意义(F=3.158,P<0.05)。PI、RI、肺超声评分与疾病严重程度呈正相关(r=0.590,0.619,0.548,均P<0.05),PSV、MFV、EDV与疾病严重程度呈负相关(r=-0.583,-0.611,-0.572,均P<0.05)。PI、RI与肺超声评分呈正相关(r=0.585,0.623,均P<0.05),PSV、MFV、EDV与肺超声评分呈负相关(r=-0.576,-0.609,-0.583,均P<0.05)。与PSV、MFV、EDV、PI、RI单项诊断相比,5项联合对呼吸窘迫综合征的诊断价值较高(P<0.05)。结论 经颅彩色多普勒超声参数对新生儿呼吸窘迫综合征有较强的诊断价值,且与肺超声评分具有一定的相关性,可用于新生儿呼吸窘迫综合征患儿病情评估及诊断。 Objective To explore the correlations between transcranial color Doppler ultrasound parameters and lung ultrasound score and the diagnostic value for neonatal respiratory distress syndrome.Methods A total of ninety-eight children suspected of neonatal respiratory distress syndrome admitted to Wenzhou People's Hospital from March 2020 to March 2022 were collected,transcranial color Doppler ultrasound was performed,after ultrasonic examination,forty-nine children diagnosed as respiratory distress syndrome definitely were included into study group,and the other forty-nine children without respiratory distress syndrome were included into control group.The transcranial color Doppler ultrasound parameters in the two groups were compared,the transcranial color Doppler ultrasound parameters in neonates with different degrees of the disease in study group were compared,lung ultrasound scores in neonates with different degrees of the disease in study group were compared,the correlations between transcranial color Doppler ultrasound parameters,lung ultrasound score and severity of the disease were analyzed,the correlations between transcranial color Doppler ultrasound parameters and lung ultrasound score were analyzed,the diagnostic significance of transcranial color Doppler ultrasound parameters for respiratory distress syndrome was analyzed.Results Peak systolic velocity(PSV),mean flow velocity(MFV),end diastolic velocity(EDV),pulsatility index(PI),resistance index(RI)in control group were(43.26±8.69)cm/s,(14.53±4.58)cm/s,(25.75±6.84)cm/s,(0.57±0.09),and(0.89±0.31),respectively.PSV,MFV,EDV,PI,and RI in study group were(33.61±6.37)cm/s,(9.07±3.46)cm/s,(18.11±5.82)cm/s,(0.71±0.11),(1.39+0.45),respectively.There were statistically significant differences in PSV,MFV,EDV,PI,and RI between the two groups(t=6.269,6.658,5.955,6.895,6.405,all P<0.05).There were statistically significant differences in PSV,MFV,EDV,PI,and RI among non-critical group,critical group,and critical emergency group(all P<0.05).Lung ultrasound scores in non-critical group,critical group,and critical emergency group were(14.56±5.93),(18.11±6.04),and(21.57±6.49),respectively,there was statistically significant difference(F=3.158,P<0.05).PI,RI,and lung ultrasound score were positively correlated with severity of the disease(r=0.590,0.619,0.548,all P<0.05).PSV,MFV,and EDV were negatively correlated with severity of the disease(r=-0.583,-0.611,-0.572,all P<0.05).PI and RI were positively correlated with lung ultrasound score(r=0.585,0.623,P<0.05).PSV,MFV,and EDV were negatively correlated with lung ultrasound score(r=-0.576,-0.609,-0.583,all P<0.05).Compared with single diagnosis of PSV,MFV,EDV,PI,RI,the diagnostic value of joint detection of five indexes was higher(P<0.05).Conclusion Transcranial color Doppler ultrasound parameters have high diagnostic value for neonatal respiratory distress syndrome,which have certain correlation with lung ultrasound score,transcranial color Doppler ultrasound parameters can be used for disease evaluation and diagnosis of neonatal respiratory distress syndrome.
