摘要
目的比较不同甲基泼尼松龙治疗方案对胆道闭锁患儿血清胆红素水平及术后胆管炎预防效果的影响。方法选取我院收治的胆道闭锁患儿114例,均接受改良Kassi术治疗,依照甲基泼尼松龙应用剂量分为小剂量组和大剂量组,每组57例。小剂量组给予小剂量甲基泼尼松龙维持治疗,大剂量组给予大剂量甲基泼尼松龙冲击治疗,对比两组患儿血清胆红素水平、黄疸消退情况、激素相关并发症情况及术后6个月的胆管炎发生情况。结果两组术后3个月总胆红素、直接胆红素水平较术前降低,且大剂量组低于小剂量组(P<0.05)。大剂量组黄疸消退情况优于小剂量组(P<0.05)。大剂量组并发症发生率与小剂量组比较,P>0.05;大剂量组术后胆管炎发生率与小剂量组比较,P>0.05。结论大剂量甲基泼尼松龙治疗可改善胆道闭锁患儿血清胆红素水平,促进黄疸消退,安全性良好。
Objective To compare the effect of different methylprednisolone treatment regimens on serum bilirubin levels and postoperative cholangitis prevention in children with biliary atresia.Methods 114 children with biliary atresia who received modified kassi treatment in our hospital were selected and divided into the low-dose group and the high-dose group,57 cases in each group.The low-dose group were given low-dose methylprednisolone maintenance treatment,and the high-dose group were given high-dose methylprednisolone impact treatment.Serum bilirubin level,jaundice resolution,hormone-related complications and cholangitis recurrence rate 6 months after surgery were compared between the two groups.Results The levels of total bilirubin and direct bilirubin in two groups 3 months after operation were lower than those before operation,and the high dose group was lower than the low dose group(P<0.05).The regression of jaundice in high-dose group was better than that in low-dose group(P<0.05).There was no difference in complication rate and incidence of postoperative cholangitis between high-dose group and low-dose group(P>0.05).Conclusion High-dose methylprednisolone therapy can improve the level of serum bilirubin,promote the regression of jaundice for children with biliary atresia.
