摘要
目的探讨门诊专科护士参与的医护一体化用药指导模式在儿童变应性鼻炎(AR)治疗中的应用效果。方法选取2022年11月—2023年6月来我院儿科门诊就诊,既往被诊断为AR,曾被处方使用过鼻用激素但未被指导过使用方法的患儿73例,随后实施以专科护士参与的医护一体化用药指导模式进行干预,观察时间为4周。采用视觉模拟量表(VAS)和儿童鼻结膜炎生活质量问卷调查表(PRQLQ)对患儿的鼻炎症状与生活质量进行评分。比较患儿的用药依从性、鼻炎症状以及生活质量。结果最终58例患儿完成随访。专科护士参与医护一体化用药指导模式对患儿进行用药指导后,患儿的用药依从率从46.6%上升至96.6%;鼻部症状总评分8(4.75,12)分,显著低于指导前的23(15,27)分,差异有统计学意义(P<0.01);眼部症状总评分1(0,4)分,显著低于指导前的4(0,9)分,差异有统计学意义(P<0.01)。在患儿PRQLQ评分中,鼻部症状困扰评分为3(1.75,4)分、眼部症状困扰0(0,1)分、生活困扰问题3(1,4)分、非鼻眼症状2(0,3)分、情感方面1(0,3)分,均较指导前显著下降,差异有统计学意义(P<0.01)。结论门诊专科护士参与医护一体化用药指导模式对AR患儿的应用效果显著,有利于提高患儿用药依从性,减轻鼻炎症状,提升患儿的生活质量。
Objective To explore the application of an outpatient integrated medication guidance model involving specialist nurses in the treatment of pediatric allergic rhinitis(AR).Methods Seventy-three children diagnosed with AR,who had been prescribed nasal steroids but had not been guided on their usage,were selected from our outpatient department from November 2022 to June 2023.An intervention involving specialist nurses in an integrated medication guidance model was implemented,with an observation period of 4 weeks.Nasal symptoms and quality of life were assessed using a visual analog scale(VAS)and the Pediatric Rhinoconjunctivitis Quality of Life Questionnaire(PRQLQ).Medication compliance,nasal symptoms,and quality of life were compared.Results Fifty-eight children completed the follow-up.After implementing the integrated medication guidance model involving specialist nurses,the medication compliance rate increased from 46.6%to 96.6%.The total nasal symptom score decreased significantly from 23(15,27)to 8(4.75,12)(P<0.01),and the total eye symptom score decreased significantly from 4(0,9)to 1(0,4)(P<0.01).The PRQLQ scores showed significant improvement in nasal symptoms(3[1.75,4]vs.4[3,6]),eye symptoms(0[0,1]vs.1[0,2]),practical problems(3[1,4]vs.4[2,5]),non-nose/eye symptoms(2[0,3]vs.3[1,4]),and emotional issues(1[0,3]vs.2[1,4])(P<0.01).Conclusion The application of an outpatient integrated medication guidance model involving specialist nurses significantly benefits AR children by strengthening medication compliance,reducing nasal symptoms,and increasing quality of life.
