摘要
目的探讨区域协同照护模式在慢性阻塞性肺疾病(慢阻肺)患者居家无创通气中的护理效果。方法选取徐州医科大学附属宿迁医院2021年12月至2022年6月收治的需要家庭无创通气的90例慢性阻塞性肺疾病患者为研究对象,随机分为观察组(n=45)和对照组(n=45)。对照组采用常规出院指导和电话随访模式,观察组在常规护理的基础上构建区域协同照护模式,建立患者的个人电子档案,线上线下指导、定期随访调查,专科护士下沉,从横向到纵向为患者提供最大限度专业指导,实现医院-社区-家庭三级联动与无缝衔接。比较两组各项指标。结果两组患者干预前无创通气依从性、肺功能、血氧饱和度(SpO_(2))、改良版英国医学研究委员会呼吸困难量表(mMRC)评分、慢性阻塞性肺疾病评估测试得分(CAT)评分、呼吸道疾病问卷(AQ20)评分等指标比较差异无统计学意义(P>0.05),干预后观察组患者的无创通气依从性、肺功能、SpO_(2)、mMRC评分、CAT评分、AQ20评分等指标明显优于对照组,差异有统计学意义(P<0.05)。结论区域协同模式对提高慢阻肺稳定期患者家庭无创通气治疗依从性效果显著,能够明显改善患者肺功能,提高其机体活动耐力,改善患者生活质量。
Objective To investigate the nursing impact of the regional cooperative care method in patients with chronic obstructive pulmonary disease receiving non-invasive ventilation at home.Methods A total of 90 patients with chronic obstructive pulmonary disease requiring home non-invasive ventilation admitted to this hospital from December 2021 to June 2022 were selected as the study objects,and were randomly divided into observation group(n=45)and control group(n=45).The control group adopted the routine discharge guidance and telephone follow-up mode,while the observation group built the regional collaborative care mode on the basis of routine care,established the patients'personal electronic files,online and offline guidance,regular follow-up investigation,and specialist nurses were immersed to provide the patients with the maximum professional guidance from horizontal to vertical,and realized the three-level linkage and seamless connection between hospital-community-family.Indicators were compared between the two groups.Results Before the intervention,there were no statistically significant differences between the two groups in terms of noninvasive ventilation compliance,lung function,Oxygen Saturation of Blood(SpO_(2)),Modified Medical Research Council Dyspnea Scale(mMRc)score,Chronic Obstructive Pulmonary Disease Assessment Test(CAT)score,and Respiratory Questionnaire-20(AQ20)(P>0.05).After intervention,noninvasive ventilation compliance,lung function,SpO_(2),mMRC score,CAT score,AQ20 score and other indexes in observation group were significantly better than those in control group,and the differences were statistically significant(P<0.05).Conclusions Regional cooperative mode has a significant effect on improving the compliance of home non-invasive ventilation treatment in COPD patients in stable stage,and can significantly improve the lung function of patients,enhance their physical activity endurance,and improve the quality of life of patients.
