摘要
目的:分析2021—2023年某三甲医院ICU病原菌分布及耐药性。方法:选取柳州市柳铁中心医院2021—2023年ICU各类送检标本中分离的1 581株病原菌作为研究资料,均行菌株鉴定和药敏试验。统计1 581株病原菌标本来源、类型、耐药性。结果:1 581株病原菌主要分离自下呼吸道标本,共分离1 195株(75.6%),类型主要是革兰阴性菌[1 200株(75.9%)],其次是革兰阳性菌[258株(16.3%)]和真菌[123株(7.8%)]。革兰阴性菌中,以鲍曼不动杆菌为主;革兰阳性菌中,以金黄色葡萄球菌为主。鲍曼不动杆菌对替加环素敏感性较高,敏感率>80%。对头孢类、β-内酰胺酶抑制剂类、碳青霉烯类、氨基糖苷类药物的耐药率均>80%。金黄色葡萄球菌对青霉素G耐药率>95%,对利奈唑胺、万古霉素、替考拉宁、替加环素和头孢洛林的耐药率均为0。结论:2021—2023年该院ICU病原菌感染以呼吸道感染为主,类型主要是革兰阴性菌,临床可对病原菌耐药性的变化进行实时监测,合理选用抗生素的同时控制和减少院内感染。
Objective:To analyze the pathogenic bacteria distribution and drug resistance in the ICU of a Grade-A tertiary hospital from 2021 to 2023.Methods:A total of 1581 strains of pathogenic bacteria isolated from various specimens submitted by ICU for testing in Liuzhou Liutie Central Hospital from 2021 to 2023 were selected as research data.All strains were identified and drug susceptibility tests were performed.The specimen sources,types and drug resistance of 1581 strains of pathogenic bacteria were counted.Results:Among 1581 strains of pathogenic bacteria,a total of 1195 strains(75.6%)were isolated from lower respiratory tract specimens.The types were mainly Gram-negative bacteria[1200 strains(75.9%)],followed by Gram-positive bacteria[258 strains(16.3%)]and fungi[123 strains(7.8%)].Gram-negative bacteria were mainly Acinetobacter baumannii;Gram-positive bacteria were mainly Staphylococcus aureus.Acinetobacter baumannii was highly resistant to tigecycline,with a sensitivity rate of>80%.The resistance rate to cephalosporins,β-lactamase inhibitors,carbapenems,and aminoglycosides was>80%.The resistance rate of Staphylococcus aureus to penicillin G was>95%,and resistance rate to linezolid,vancomycin,teicoplanin,tigecycline,and ceftaroline were all 0.Conclusion:From 2021 to 2023,the main pathogenic bacterial infections in the ICU of this hospital are respiratory tract infections,and the types are mainly Gram-negative bacteria.In clinical setting,it is necessary to perform real-time monitoring of the pathogenic bacterial resistance change and rationally use antimicrobial drugs,so as to control and reduce nosocomial infection.
