摘要
目的:分析闭合复位髓内钉内固定术治疗老年肱骨外科颈骨折的效果。方法:选取2020年2月—2023年2月公安县中医医院收治的肱骨外科颈骨折患者90例作为研究对象,采用随机数字表法分为对照组与试验组,各45例。对照组实施切开复位内固定术,试验组实施闭合复位髓内钉内固定术。比较两组治疗效果。结果:试验组手术时间及骨折愈合时间短于对照组,术中出血量少于对照组,手术切口小于对照组,差异有统计学意义(P<0.001)。术后3个月,两组肩关节后伸、外旋、内旋、外展、前屈评分高于术前,且试验组高于对照组,差异有统计学意义(P<0.05)。试验组并发症总发生率低于对照组,差异有统计学意义(P=0.014)。结论:闭合复位髓内钉内固定术治疗老年肱骨外科颈骨折的效果显著,手术时间短、切口小、失血量少,术后患者骨折端能够快速愈合,肩关节恢复情况较好,并发症发生率低。
Objective:To analyze the effect of closed reduction and intramedullary nail internal fixation in treatment of elderly humeral surgical neck fracture.Methods:From February 2020 to February 2023,90 patients with humeral surgical neck fractures who were admitted to Gongan Hospital of Traditional Chinese Medicine were selected as the study subjects.According to a random number table method,the patients were divided into a control group and an experimental group with 45 cases in each group.The control group underwent open reduction and internal fixation,while the experimental group underwent closed reduction and intramedullary nail internal fixation.The treatment effects of the two groups were compared.Results:The operation time and fracture healing time in the experimental group were shorter than those in the control group,the intraoperative blood loss in the experimental group was less than that in the control group,the surgical incision in the experimental group was smaller than that in the control group,and the difference was statistically significant(P<0.001).At 3 months after surgery,the scores of extension,external rotation,internal rotation,abduction and anterior flexion of the shoulder joint in the two groups were higher than those before surgery,and the scores in the experimental group were higher than those in the control group,with a statistically significant difference(P<0.05).The total incidence of complications in the experimental group was lower than that in the control group,and the difference was statistically significant(P=0.014).Conclusion:The closed reduction and intramedullary nail internal fixation has significant effects in the treatment of elderly humeral surgical neck fracture.It has short operation time,small incision and few blood loss.After surgery,the fracture end of the patient can heal rapidly,the shoulder joint recovery is better,and the complication rate is low.
