摘要
目的分析2020年-2022年喀什地区某医院肺结核患者的住院费用构成情况并探讨其影响因素,为减轻患者疾病经济负担提供证据参考。方法选取喀什地区某肺结核专科医院2020年1月-2022年12月间诊治的9570名肺结核住院患者作为研究对象,运用描述性分析、非参数检验、多元线性回归分析对肺结核患者住院费用及影响因素进行分析。结果肺结核患者住院费用中位数(四分位数)为13150.446(9891.34,19648.478)元;在住院费用构成中,材料费(30.25%)和药品费(24.01%)占比最高。影响肺结核患者住院费用的主要因素为年龄、婚姻状况、住院天数以及初治情况(均P<0.001)。结论优化住院费用结构,进一步降低材料费和药品费;完善相关医保政策,扩大医疗保障范畴;健全规范化诊疗和管理模式,合理缩减住院天数;加强对肺结核防治知识的科普宣传是有效控制肺结核患者住院费用的关键。
Objective To analyze the composition of hospitalization expenses of patients with pulmonary tuberculosis(PTB)in a hospital of Kashgar from 2020 to 2022 and to explore its influencing factors,so as to provide evidence for reducing the economic burden of patients.Methods 9570 hospitalized patients with pulmonary tuberculosis diagnosed and treated at a tuberculosis specialized hospital in Kashgar from January 2020 to December 2022 were selected as the research subjects.Descriptive analysis,non parametric testing,and multiple linear regression methods were used to analyze the hospitalization expenses and influencing factors.Results The median(quartile)of total hospitalization expenses for patients with pulmonary tuberculosis is 13150.446(9891.34,19648.478)yuan.Among the composition of hospitalization expenses,material expenses(30.25%)and drug expenses(24.01%)accounted for the highest proportion.The main factors affecting the hospitalization expenses were age,marital status,hospitalization days,and initial treatment(all P<0.001).Conclusion It is necessary to optimize the structure of hospitalization expenses and further reduce material and drug costs,improve relevant medical insurance policies and expand the scope of medical insurance,improve standardized diagnosis,treatment,and management models,and reasonably reduce hospitalization days.Strengthening scientific popularization and promotion of pulmonary tuberculosis prevention and control knowledge are the key to effectively controlling the hospitalization expenses of patients with pulmonary tuberculosis.
