多模态联合技术在甲状腺结节诊断中的临床应用价值

摘要 目的:探讨多模态联合技术在甲状腺结节诊断中的临床应用价值为临床诊断提供科学依据。方法:2018年1月1日—2022年12月31日,选取在甘肃省武威肿瘤医院就诊的甲状腺结节患者258例,分析多模态联合技术对甲状腺结节良恶性的临床诊断价值。结果:258例患者中,甲状腺恶性结节患者的最大直径明显小于良性结节患者的最大直径;在超声诊断的基本特征比较中,良性结节患者与恶性结节患者的纵横比、内部回声、边界形态、钙化程度、血流分布比较,差异均有统计学意义(χ^(2)值分别为9.664、15.949、101.347、15.607、7.583,P值均<0.05);病理活检确诊为良性结节患者与恶性结节患者的常规超声、实时超声弹性评分比较,差异均有统计学意义(χ^(2)值分别为161.55、99.361,P值均<0.01)。常规超声+实时超声弹性评分诊断甲状腺良恶性结节效能最优、灵敏度最高。结论:常规超声联合实时超声弹性评分技术对甲状腺结节良恶性诊断在临床中具有重要意义,可在医疗条件相对薄弱的地区推广和应用。 OBJECTIVE To investigate the clinical application value of multimodal combined technique in the diagnosis of thyroid nodules.METHODS From January 1,2018 to December 31,2022,a total of 258 patients with benign and malignant thyroid nodules diagnosed by ultrasound-guided fine needle biopsy or surgical pathological biopsy were selected from Wuwei Cancer Hospital of Gansu Province.The results of thyroid surgical pathological biopsy or ultrasound-guided fine needle biopsy were taken as the gold standard,to analyze the value of multimodal combined technique in the diagnosis of benign and malignant thyroid nodules.RESULTS In 258 patients,the maximum diameter of patients with malignant thyroid nodules was significantly smaller than that of patients with benign nodules.In the comparison of the basic characteristics of ultrasound diagnosis,there were statistically significant differences in the aspect ratio,internal echo,boundary shape,calcification degree and blood flow distribution between benign and malignant nodules(χ^(2)=9.664,15.949,101.347,15.607,7.583,P<0.05).There was significant difference between the patients with benign nodules confirmed by pathological biopsy and the patients with malignant nodules by routine ultrasound and real-time ultrasound elasticity scores(χ^(2)=161.55,99.361,P<0.01).Conventional ultrasound plus real-time ultrasound elastic score is the most effective and sensitive method in the diagnosis of benign and malignant thyroid nodules.CONCLUSION Conventional ultrasound technology combined with real-time ultrasound elastic scoring technology has high efficiency and sensitivity in the diagnosis of benign and malignant thyroid nodules,which is of great significance in clinical diagnosis,and can be popularized and applied in areas with relatively weak medical conditions.
