二极管激光辅助治疗种植体周围疾病的Meta分析

Efficacy of Diode Laser in the Adjuvant Treatment of Peri-Implant Diseases:A Meta-Analysis

摘要 为系统性评价二极管激光辅助治疗种植体周围疾病的临床疗效,检索中国知网、万方、维普和中国生物医学文献数据库、PubMed、Embase、Cochrane Library等数据库中关于二极管激光辅助治疗种植体周围疾病的文献,检索时间从建库至2022年6月。由2位评价者独立鉴别、分析、评估相关文献后采用RevMan5.4进行Meta分析。最终筛选纳入文献8篇,Meta分析结果显示:相较于单纯机械清创治疗,二极管激光辅助治疗种植体周围疾病能显著减少治疗后3个月的牙周探诊深度(PD)[MD=-0.28,95%CI:(-0.46,-0.10),P=0.002],能有效减少3个月后的菌斑指数(PI)[MD=-0.13,95%CI:(-0.21,-0.05),P=0.002],能明显减少3个月后的出血指数(BI)[MD=-0.42,95%CI:(-0.83,-0.01),P=0.05],也能改善治疗3个月后的探诊出血指数(BOP)[MD=-1.81,95%CI:(-2.53,-1.08),P<0.00001]。但治疗6个月后,两组PD、PI、BI比较均不存在显著差异(P>0.05)。现有证据表明,二极管激光辅助治疗种植体周围疾病仅在3个月的短期观察随访中有效。 Objective:This study systematically evaluates the clinical efficacy of diode laser as an adjuvant treatment for peri-implant diseases.Methods:A comprehensive search of literature on diode laser treatment for peri-implant diseases was conducted across various databases.Meta-analysis was carried out using RevMan 5.4software after independent reviewers screened the literature,assessed study quality,extracted data,and evaluated bias risk in the included studies.Results:8papers were finally screened and included.The meta-analysis showed that compared with the mechanical debridement group,probing depth(PD)[MD=-0.28,95%CI:(-0.46,-0.10),P=0.002],plaque index(PI)[MD=-0.13,95%CI:(-0.21,-0.05),P=0.002],bleeding index(BI)[MD=-0.42,95%CI:(-0.83,-0.01),P=0.05],and bleeeding on probing(BOP)[MD=-1.81,95%CI:(-2.53,-1.08),P<0.00001]had significcant statistical difference between diode laser combined with mechanical debridement group and control group after 3months of treatment.However,after 6months of treatment,there was no significant difference in PD,PI and BI between the two groups(P>0.05).Conclusion:The current evidence suggests that diode laser-assisted treatment for peri-??h?implant diseases shows effectiveness only in the short term,with significant improvements observed after 3months of treatment.
