摘要
目的:探讨鞘内注射吗啡在老年重度癌性疼痛患者中的应用效果。方法:选取2021年1月—2022年1月泰安市肿瘤防治院收治的90例老年重度癌性疼痛患者作为研究对象,采用随机数字表法分为硬膜外组和鞘内组,各45例。硬膜外组实施硬膜外注射吗啡镇痛,鞘内组实施鞘内注射吗啡镇痛。比较两组治疗效果。结果:镇痛后7、14、30d,两组每日吗啡平均用量低于镇痛前,且鞘内组低于硬膜外组,差异有统计学意义(P<0.05)。镇痛后24h,两组运动状态下及静息状态下癌性疼痛评分均低于镇痛前,且鞘内组低于硬膜外组,差异有统计学意义(P<0.05)。镇痛后1个月,鞘内组不良反应总发生率低于硬膜外组,差异有统计学意义(P=0.003)。结论:鞘内注射吗啡在老年重度癌性疼痛患者中的应用效果显著,可减少吗啡用量,提高镇痛效果,降低不良反应发生率。
Objective:To explore the application effects of intrathecal morphine injection in elderly patients with severe cancer pain.Methods:A total of 90 elderly patients with severe cancer pain who were treated in Tai'an Tumor Prevention and Treatment Hospital from January 2021 to January 2022 were selected as the study subjects.They were divided into epidural group and intrathecal group by random number table method,with 45 patients in each group.The epidural group received epidural morphine injection for analgesia,and the intrathecal group received intrathecal morphine injection for analgesia.The treatment effects were compared between the two groups.Results:The average daily dose of morphine in the two groups on 7 d,14 d and 30 d after analgesia was lower than that before analgesia,the dose in the intrathecal group was lower than that in the epidural group,and the difference was statistically significant(P<0.05).At 24 h after analgesia,the scores of cancer pain under exercise state and resting state in two groups were lower than those before analgesia,the scores in the intrathecal group were lower than those in the epidural group,and the difference was statistically significant(P<0.05).At 1 month after analgesia,the total incidence of adverse reactions in the intrathecal group was lower than that in the epidural group,and the difference was statistically significant(P=0.003).Conclusion:Intrathecal morphine injection has significant application effects in elderly patients with severe cancer pain and can reduce morphine dose,improve analgesia effect,and decrease the incidence of adverse reactions.
