摘要
为探究冲击波冲击次数、压力、频率等对脑卒中后患者上下肢痉挛的影响,总结冲击波治疗卒中的良好参数,采用文献分析法与数理统计法,在中国知网、万方、维普、Pubmed等数据库中以“ESWT”“Extracorporeal shock wave”“Stroke”“Spasticity”进行相互匹配以检索相关文献。在冲击次数及频率方面,上下肢的治疗方案二者具有高度重合性,故认为冲击波在进行卒中后痉挛的选择中,2000次冲击方案与8 Hz频率对于患者来说是高效且安全的参数,冲击波在卒中后痉挛的治疗中具有良好效果;上肢的冲击波使用压力为2.0~3.0 bar,下肢常在1.5~2.0 bar,但仍需探讨其合理性;冲击波使用频率在上下肢中普遍认可的参数为8 Hz;单次治疗击打次数普遍认可的参数为2000次。需要更为高质量、随机化的实验进行相关效果验证,建议挑选合适的卒中后痉挛受试者进行上/下肢不同压力参数的临床对照实验,从而进一步了解压力的选择。在实验评价中应加入更加客观的评价指标,为今后临床提供指导。
In order to explore the influence of the frequency,pressure and the frequency of shock wave impact on the upper and lower limb spasticity of patients after stroke,and summarize the good parameters of stroke wave treatment,the study uses literature analysis method and mathematical statistics method,and matches“ESWT”“Extracorporeal shock wave”“Stroke”“Spasticity”in CNKI,Wanfang,VIP and Pubmed databases,and searches relevant literature.In terms of impact frequency and frequency,the treatment plans of the upper and lower limbs are highly overlapping.For this reason,it can be considered that the frequency of 2000 shocks and 8 Hz are highly efficient and safe parameters for patients.Shock wave has good effect in the treatment of post-stroke Spasticity.Shock wave pressure of the upper limbs is within 2.0~3.0 bar,the lower limbs are often in 1.5~2.0 bar.So it still needs to explore its rationality.The shock wave usage frequency in the upper and lower limbs is 8 Hz.The number of shock wave strikes in the upper and lower limbs is 2000 times.The reliability of some literature still needs to be confirmed.It is suggested to select suitable subjects with spasm after stroke to conduct clinical controlled experiments with different pressure parameters of upper and lower limbs,so as to further understand the choice of pressure.In the experimental evaluation,more objective evaluation indicators should be added.
