摘要
目的:探究聚焦解决模式对骨科大手术患者围术期负性情绪和应对能力的影响。方法:选取2023年3-5月在滨州医学院附属医院需进行人工全髋关节置换术、人工全膝关节置换术和髋部骨折手术的患者88例为研究对象,采用随机数字表法分为对照组和试验组,各44例。对照组采用常规护理,试验组在对照组基础上给予聚焦解决模式心理干预。比较两组医院焦虑抑郁量表(HADS)、中文版医学应对问卷、功能锻炼依从性量表、出院患者护理满意度评分。结果:干预后,试验组HADS评分降低,且低于对照组,差异有统计学意义(P<0.05)。干预后,试验组面对评分升高,且高于对照组,差异有统计学意义(P<0.05);两组干预前后屈服评分比较,差异无统计学意义(P>0.05);干预后,试验组回避评分降低,且低于对照组,差异有统计学意义(P<0.05)。试验组功能锻炼依从性得分高于对照组,差异有统计学意义(P=0.003)。试验组护理满意度得分高于对照组,差异有统计学意义(P=0.019)。结论:聚焦解决模式运用于骨科大手术患者围术期心理干预,可以明显改善骨科大手术患者围术期焦虑、抑郁情绪,提高患者医学应对能力、功能锻炼依从性及满意度。
Objective:To explore the effect of focused resolution mode on perioperative negative emotion and coping ability in pa-tients undergoing major orthopedic surgery.Methods:A total of 88 patients requiring total hip arthroplasty,total knee arthroplasty and hip fracture surgery in the Affiliated Hospital of Binzhou Medical College from March to May 2023 were selected as the study objects,and were divided into control group and experimental group by random number table method,with 44 patients in each group.The control group received routine nursing,and the experimental group received focused solution mode psychological inter-vention on the basis of the control group.The hospital Anxiety and Depression Scale(HADS),Chinese Version of Medical Re-sponse Questionnaire,Functional Exercise Compliance Scale and nursing satisfaction score of discharged patients were compared between the two groups.Results:After intervention,the HADS score of the experimental group was lower than that of the control group,and the difference was statistically significant(P<0.05).After intervention,the response score of experimental group was higher than that of control group,and the difference was statistically significant(P<0.05);there was no significant difference in yield score between the two groups before and after intervention(P>0.05);after intervention,the avoidance score of the experi-mental group was lower than that of the control group,and the difference was statistically significant(P<0.05).The score of func-tional exercise compliance in experimental group was higher than that in control group,and the difference was statistically signifi-cant(P=0.003).The nursing satisfaction score of experimental group was higher than that of control group,and the difference was statistically significant(P=0.019).Conclusion:The application of focused solution mode in perioperative psychological interven-tion can significantly improve the perioperative anxiety and depression of patients with major orthopaedic surgery,and improve the patients'medical response ability,functional exercise compliance and satisfaction.
