摘要
脑血管相关性疾病是中年及以上群体的常见病、多发病,严重威胁着人们的健康。现代医学主要以降血脂、调节血压、调节血糖、抗血小板聚集、营养神经、外科手术等为主要治疗方案,用药繁多、疗程长,易产生耐药性及不良反应,且存在手术禁忌症。涤痰汤是治中风痰迷心窍、舌强不能言的经典方剂。查阅并梳理近年相关文献发现涤痰汤治疗脑血管相关性疾病具有疗效好、不良反应少、安全性高等优势;其作用机制包括促进神经因子表达、调控兴奋性氨基酸毒性、抗炎、抗氧化应激、调节自噬、抗凋亡、调节血脂、促血管新生等作用。该文梳理了涤痰汤治疗脑血管相关性疾病的相关研究,发现涤痰汤治疗脑血管相关性疾病已取得了较大进展,但仍存在一些不足:如部分研究为综合疗法,涤痰汤的具体疗效不明确;仍缺乏多中心、大样本、随机双盲的高质量随机对照试验;部分研究仍在试验阶段,其作用于人体的安全性、有效性仍待验证;后续仍需深入机制研究,将方药与现代科学联系起来,明确方药的具体作用,挖掘新的治疗思路。
Cerebrovascular-related diseases are common and frequent among middle-aged people and above,and seriously threaten people's health.Modern medicine mainly takes lowering blood lipids,regulating blood pressure,regulating blood sugar,anti-platelet aggregation,nerve nutrition and surgery as the main treatment options,with numerous drugs,long treatment courses,that's easy to produce drug resistance and adverse reactions,and contraindications to surgery.The classical formula for the treatment of stroke with phlegm confusing the heart and a strong tongue that is unable to speak.A review of the relevant literature in recent years showed that it has good efficacy,low adverse effects and high safety in the treatment of cerebrovascular-related diseases.Its mechanism of action includes promoting neurofactor expression,regulating excitatory amino acid toxicity,anti-inflammation,anti-oxidative stress,regulating autophagy and anti-apoptosis,regulating blood lipids,promoting vascular renewal and other effects.In this paper,we have combed through the studies on the treatment of cerebrovascular-related diseases,and found that the treatment of cerebrovascular-related diseases with Ditan Decoction(涤痰汤)has made great progress,but there are still some shortcomings:for example,some of the studies are comprehensive therapies,and the specific efficacy of Ditan Decoction is not clear,there is still a lack of multicenter,large sample,randomized double-blind high-quality randomized controlled trials;some of the studies are still in the experimental stage,and the safety and efficacy of its action in humans still need to be verified;the follow-up is still in progress,some of the studies are still in the experimental stage,and their safety and efficacy in humans still need to be verified;the follow-up still needs in-depth mechanism research,linking the prescriptions with modern science,clarifying the specific effects of the prescriptions,and exploring new therapeutic ideas.
