摘要
目的:探讨糖尿病肾病患者血清甲状腺激素、糖化血红蛋白(HbA1c)及肾损害相关指标表达水平及相关性.方法:选取2019年12月至2022年12月本院收治的68例糖尿病肾病患者作为研究对象,将其纳入糖尿病肾病组.另选取同期本院收治的82例2型糖尿病患者纳入单纯糖尿病组,以及在本院进行健康体检的72例健康受试者纳入对照组.比较3组血清甲状腺激素、HbA1c与肌酐、尿酸、24h尿蛋白、胱抑素C水平,并分析其相关性.结果:糖尿病肾病组游离三碘甲状腺原氨酸(FT3)水平,明显低于单纯糖尿病组和对照组,促甲状腺激素(TSH)水平,明显高于单纯糖尿病组和对照组,差异均有统计学意义(P<0.05);3组HbA1c、肌酐、尿酸、24h尿蛋白、胱抑素C水平比较,糖尿病肾病组上述指标水平,均明显高于单纯糖尿病组及对照组,差异均有统计学意义(P<0.05);糖尿病肾病组FT3水平与HbA1c、肌酐、尿酸、24h尿蛋白、胱抑素C水平,均呈负相关关系(r<0,P<0.05).结论:糖尿病肾病组患者FT3水平降低与HbA1c、肌酐、尿酸、24h尿蛋白、胱抑素C呈负相关性,FT3可作为评估糖尿病患者肾损害的指标之一.
Objective:To investigate the level and correlation of serum thyroid hormone,glycosylated hemoglobin(HbAlc),and renal damage related indicators in patients with diabetes nephropathy.Methods:A total of 68 patients with diabetes nephropathy treated in our hospital from December 2019 to December 2022 were selected in this study and included in the diabetes nephropathy group.In addition,82 patients with type 2 diabetes who admitted to our hospital in the same period were selected in the simple diabetes group,and 72 cases healthy subjects who cook physical examination in our hospital during the same period were selected as control group.The levels of serum thyroid hormone,HbAlc,creatinine,uric acid,24h albuminuria,and cystatin C were compared among 3 groups and their correlations were analyzed.Results:The level of free triodothyronine(FT3)in diabetic nephropathy group was significantly lower than those in simple diabetic group were control group,and the hormothyrin(TSH)level of diabetic nephropathy group was significantly higher than those of simple diabetic group and control group,there were statistically significant differences(P<0.05).The levels of HbAlc,creatinine,uric acid,24-hour urinary protein,cystatin C in diabetic nephropathy group were significantly higher than those in simple diabetic group and control group,there were statistically significant differences(P<0.05).The levels of FT3 were negatively correlated with the levels of HbAlc,creatinine,uric acid,24h albuminuria and cystatin C in diabetic nephropathy group(r<0,P<0.05).Conclusion:In diabetic nephropathy patients,FT3 level were negatively correlated with HbAlc,creatinine,uric acid,24h albuminuria and cystatin C.The results indicated that FT3 could be used as an indicator to evaluate renal damage in diabetic patients.
